IMPLANTOLOGY

Implantology is the epitome of restoration dentistry, ensuring the functional and aesthetical replacement of lost teeth. Long term studies show that implants, when properly placed and cared for, enjoy a lifespan comparable to that of natural teeth. Hence it is no wonder that implantology continues to gain prominence throughout the world of dentistry.

Titanium alloy - The material of choice

All SGS Dental Systems' implants are made of the titanium alloy Ti-6Al-4V ELI, in accordance with ASTM-F136-02. Titanium has proven to be the ideal implant material for many reasons, perhaps the most important being its ability to integrate almost completely with the bone. In addition to being «bio-friendly» it enjoys favorable mechanical qualities (strength, endurance) and can be precisely fabricated (precision measured in microns) to ensure a range of implants that meet the requirements for optimizing stability in the widest range of patients (taking into account the dimensions and state of health of an individual’s bone and gums).

After 30 days the osteocalcin –expression too was significant increased by the implants with the BONITex-surface.

SterilitySGS Dental Systems’ implants undergo a strict sterilization process, including exposure to Gamma irradiation (at a level of 2.5 mrad) as stipulated in «Sterilization of medical devices - Validation and routine control of sterilization by irradiation - EN 552 (1994)”. Samples are tested for sterility using USP 24 procedures for medical devices (membrane filtration method). Irradiated finished products also undergo «Bio-Burden test - ISO 11637-1» to assure sterilization.

fatigue testingThe mechanical strength (fatigue test) of SGS Dental Systems’ implants was tested according to the “Class II Special Control Guidance Document: Root form Endosseous Implants and Abutments: Draft Guidance for Industry and FDA” at the Metallurgical Lab of the «Technion Research and Development Foundation». These implants were found to be highly resistant to fatigue under extreme mechanical loading forces.

restoration Options

SGS Dental Systems offers the widest range of tools and devices for oral restoration, ensuring that dental solutions can be offered to meet your patient's needs. Prosthetic accessories are suitable for all the implants with an internal hex. The abutments are available in two platforms – narrow (for implants with diameters of 3.25, 3.75 & 4.2mm) and wide (for implants with diameters of 4.5, 5 & 6 mm). All of the abutments have internal threading, which ensures that the fit will not loosen, strengthens the neck of the abutment and allows their removal by friction. Due to the precise interface between the implant and the abutment, which are designed to withstand the pressures in the mouth, the result is a very high rate of success of the implants over the years.Prior to the restoration in two-piece implants (P1 & P7), remove the cover screw or healing cap. Complete the screwing motion with a torque wrench (R8) up to 30 Ncm. (If you choose the P9S or P7S abutments with a secondary upper screw, tighten it using up to 20 Ncm.)

Double Platform Implants* - The Complete fit!

SGS Dental Systems, Ltd. offers a patented solution for a bridge which is screwed onto non-parallel implants. SGS Dental Systems' Research and Development team has developed an implant with a Flat Connection platform. The implant looks the same as all other SGS Dental Systems' implants from the outside. Inside, however, are two platforms.

The original deep hexagonal platform remains for connection to regular abutments, but a second shallow and round platform is also found inside which facilitates the Complete Fit of a Flat Connection abutment to the implant. The Flat Connection abutment is placed inside and screwed into the implant instead of a regular abutment. This new technology allows for a shorter «Path of Insertion» for the placement of the abutment.

Development of this new generation of implants will maximize surgical options without concern for the possible limitations of the future rehabilitation stage. If the reconstruction dentist decides to carry out the rehabilitation on a single implant, a regular abutment can be used. If he decides to carry out the rehabilitation following a series of multiple divergent implants, however, a Complete Fit can be achieved through the use of our Flat Connection Abutments.

Super Lock®

When an abutment is annexed to an implant using a screw it generates internal forces: the screw is stretched under tension (force T) when the abutment is forced downwards (Force Fh) by the head of the screw, and the threading action pulls the implant upwards (Force Ft). Thus the screw causes interactive forces (Fr) between the conical ring at the bottom of the abutment and the conical crater on the upper part of the implant (Figure 1). This structure resists pure vertical.

Figure 2 illustrates how the implant is supposed to react to the pressures generated by side loading of the abutment. Integrative forces F1& F2 balance the side force F. Hence the hex on the bottom of the abutment needs to precisely match the hex hole of the implant. Any margin will cause the tightening screw to bear a heavy burden and it will either bend or be lengthened, and the abutment will become loose.

Short ImplantsThese short implants have proven to be very successful. Long ImplantsThese extra long Screw Type Implants come in lengths of 18mm and 20mm. These implants are offered in the 4.2mm diameter of the P1 Screw Type Implant: P1-4.2,18 and P1-4.2,20.

However, as some margin is required, since the hex of the abutment must slide into the hole of the implant, we offer the Super Lock system. The external dimension of the Super Lock abutment neck is slightly larger than the internal dimension of the implant's hexagonal receptacle part. Nevertheless since the hexagonal neck of the Super Lock abutment is gently sliced longitudinally by tiny radial spaces in its sides, the slices gently bend inward when theabutment is forced into the implant, thereby decreasing its external dimension and allowing it to enter the 'poky' hexagonal receptacle part. When the abutment is manually inserted into its place it remains steady, reserving two free hands for tightening the screw and/or anything else that may be required. A tight fit is created between the internal walls of the implant's hexagonal receptacle part and the external walls of the abutment's hexagonal neck when the affixing screw is tightened.

Super Lock Abutment removal

Release of the abutment, which is firmly fixed in the holeof the implant, requires the use of a special tool, becauseeven after opening the screw the abutment remains tightly bound to the implant. (Even the application of normative force will not free it.) After the screw which secured the abutment is removed, the extraction screw (K7) is used (as shown in fig. 4) to release the abutment. The heads of the tightening screw and extraction screw are identical and therefore the same wrench is used for both. After the abutment is released the extraction screw must be removed and the tightening screw returned to its place.Strict precision demands (within 5 microns) both for the implant hole and the neck of the abutment, ensure the long-term, problem-free connection between them.

P9 ImplantsThe P9 is a thin implant with a replaceable ball attachment on top. It is the only thin implant available on the market with interchangeable prosthetic parts. Available in diameters of 3mm & 3.2mm. After good surgical exposure of the bony surface the position for the implant should be determined and a guide hole should be made using our round head burr taken down into the cortical bone to the level of the neck beneath the burr head. Do not attempt to drill deeper with the round burr. Using the guide hole for position, the color-coded drill bits will be utilized to drill the hole to the desired depth. The color-coding on the bits indicates the diameter of the bit. Almost all drilling (excluding all P9S implants, P7-3.75 & P7S-3.75) should commence using the 2.0 mm bit. The bits are used in graduating order to slowly increase the diameter of the implant hole until the desired diameter is reached. This will allow a safe progression and decrease trauma to the surrounding bony structures. The accurate depth of the hole is determined by the length of each particular implant and is indicated by the depth lines around each bit, in order to allow good position of the implant in the bone so that its end is flush with the alveolar ridge.

P1 Protocol

The final diameter of the hole should be one half millimeter smaller than the implant diameter (e.g. for an implant with a diameter of 3.75 mm, the final bit size would be 3.2 mm). Table 1 below summarizes the final color-coded drill for each implant.

P7, P7S Protocol

The bottom part of P7S is identical to P7, thus they share the same instructions. The best conical hole for the planned conical implant is achieved by using the appointed conical drilling bit. All bits, with the exception of the final regular bit, are inserted in turn till the required depth line reaches the alveolar ridge. The final regular bit is inserted lightly to a depth of only 2mm. The drilling protocols of tapered holes for different implants are presented in Table 2.

The most efficient method of drilling has been found to be achieved through the use of conical drilling bits. We highly recommend that our customers acquire the conical drilling bits. The conical drilling bit for each diameter is suitable for every implant length in that diameter!Where the conical drilling bits are not available, it is possible to achieve the desired tapering of the hole by redrilling with two slighter larger bits taken down to only a partial depth. The first bit, slightly larger than the bit used to reach the desired depth of the implant hole, drills only ~2/3 of the total depth and the second, slightly larger than the first, drills only ~1/3 of depth, thus creating a staged or conical tapered hole. The drilling protocols of tapered holes for different implants are presented in Table 3.

Integral Groovy Implant: (Smart Implant) One-piece implant combining implant and abutment in conical form. Suitable for immediate loading where there is enough bone in both dimensions (height and width) as in post-extraction sockets. Advantages: quick insertion; increased strength due to unified structure; ideal for non-surgical approach; works as a closed system without gum incisions, reducing risk of infections. Their optimal usage is for mild and dense bone and they are recommended for use with soft bones. The restoration componentsare limited to casting only. Thin integral implant: a thin, conical, one-piece implant combining implant and abutment. Intended for narrow alveolar ridges with sufficient depth. Appropriate for immediate loading.Advantages: suitable for use on narrow ridges, or between closely spaced teeth or implants and of course between permanent implants for temporary restoration.Their optimal usage is for mild and dense bones and they are optional for use with soft bones. Plastic structure for casting with no internal threading.

Available with or without a hex, in switch platform.

The sleeve with the hexagon is meant to build structure on implant or to set a crown or bridge that fits on to the abutment.

The sleeve without the hexagon is designated for various tasks (root balls, various attachments). The plastic sleeve is effective when a number of prosthetic devices must be fitted in. It is easy to work with because it is made of plastic and easy to cast. Its drawback: hex inaccuracy. The plastic sleeve is made from Delrin, a material appropriate for casting as it melts when heated without leaving any residue.

This unique Flat Connection Abutment fits the shallow platform of our innovative Double Platform Implants. It is suitable for use with a screw-retained denture to be fitted over multiple non-parallel implants. Conical Drilling BitsThe drill bits are internally cooled. The diameter of each drill is color-coded as well as laser-etched on the upper portion of the drill. The depth markings are both milled and laser-etched. Intended for use with conical implants. Caution!All conical drill bits are characterized by drilling through the bone along the entire length of the drill that is positioned inside the gums. This is as opposed to the regular drill bit, which only drills through the bone using the frontal lower tip. At the same time, its side helical blades slide along the wall of the hole without any significant radial forces.The use of conical drill bits causes extreme radial pressure, creating the necessity for gentle, probing drilling instead of constant drilling. This gradual drilling should include the use of a low torque and low rpm and intensive irrigation.Pausing periodically during the drilling allows both the blade and the bone to cool down. It also allows for the removal of bone fragments, as well as the control necessary at the appropriate rpm.

P7, P7S Protocol

The bottom part of P7S is identical to P7, thus they share the same instructions. The best conical hole for the planned conical implant is achieved by using the appointed conical drilling bit. All bits, with the exception of the final regular bit, are insertedin turn till the required depth line reaches the alveolar ridge. The final regular bit is inserted lightly to a depth of only 2mm.The drilling protocols of tapered holes for different implants are presented in Table 2.

The most efficient method of drilling has been found to be achieved through the use of conical drilling bits. We highly recommend that our customers acquire the conical drilling bits. The conical drilling bit for each diameter is suitable for every implant length in that diameter!Where the conical drilling bits are not available, it is possible to achieve the desired tapering of the hole by redrilling with two slighter larger bits taken down to only a partial depth. The first bit, slightly larger than the bit used to reach the desired depth of the implant hole, drills only ~2/3 of the total depth and the second, slightly larger than the first, drills only ~1/3 of depth, thus creating a staged or conical tapered hole. The drilling protocols of tapered holes for different implants are presented in Table 3.
