We strive to empower the community to take charge of their health by providing free and accessible health education, physical activity classes, and community outreach intervention.
Demographics • Hispanic – 53.3%
– Predominantly of Puerto Rican decent
• Black – 40.8%
• White – 4.44%
• Asian – 0.39%
• Other – 0.15%

The role DEC plays in the Humboldt Park Community; WHY AND HOW
• A survey study conducted by Sinai Urban Health Institute (SUHI) in 2006 concluded that 21% of Puerto Ricans in Humboldt Park were living with diabetes. •The mortality rate in Humboldt Park was 3 times higher than the U.S. •In 2010 NIH granted Rush University 2 million dollars to launch an initiative; in collaboration with SUHI they conducted a 3 study called Block by Block •The Greater Humboldt Park Community Diabetes Empowerment Center journey begins.

Healthy Cooking Demonstrations Bi-weekly and Special Events

• Six-week health program lead by Loyola medical students
• Weekly presentations on various health topics
