WITH THE FOLLOWING ENCLOSURES:

(i) Photo copies of Certificates and Testimonials or documentary evidence of qualification (ii) Four recent passport-size photographs, three of which should be affixed to the three

Application forms.(iii) The names entered on this form must be of the same spelling and order as those used on

your certificates.(iv) Completed Application Forms should be accompanied by Bank Pay-in-slips, three (3)

copies of certificates and transcripts of Academic Record (where applicable).

D. IMPORTANT NOTES:

i. Applicants must pay the cost of Application Forms into any networked branch of GCB Bank using the following details:
