What is an HSA?
• A Health Savings Account (HSA) is a tax-advantaged checking account that works in conjunction with your CDHP.
• Allows you to set aside pre-tax or post-tax dollars which can be used for any qualified medical expenses, such doctor’s visits, hospital stays, prescriptions, dental, vision, etc.
• “Use it or Keep It”. HSA dollars rollover each plan year. And goes with you in the event you retire or leave the University.
• Qualified employees will complete online application during the open enrollment period

Are You Eligible for an HSA?
IRS Eligibility Guidelines:You enroll in a qualified consumer driven health plan (CDHP) either through DePauw or your spouse. Additionally: Cannot be covered through a PPO plan Cannot be covered through Tri-care Cannot be enrolled in Medicare Parts A or B Cannot be participating in a medical FSA You cannot be claimed as a dependent on someone
else's tax return.

HSA IRS Maximums
• Maximums should also take into account any contributions made by DePauw University
• Spouses combined cannot go over the IRS limits per calendar year.
• If you or your spouse is age 55 or older and HSA-eligible, you may each contribute to your own HSA account an additional $1,000 catchup contribution.

HSA Contribution Redesign
If you enroll in one of the three CDHP plans for January 1, 2019, and qualify to make and receive HSA contributions, DePauw will make a base contribution to your HSA based on the tier of coverage you are enrolled in.
The base contribution will be available mid January, 2019.
IMPORTANT: Your health savings account must be opened before any contributions are made.
Employee Only Coverage: $250
Employee + 1 or more Dependents: $500

• You will have an opportunity to earn additional HSA contributions through a new HSA incentive plan.
• The maximum amount of HSA incentive dollars you can earn each year is based on your tier of coverage in the medical plan.
o Employee Only: $750o Employee & 1 or more Dependents: $1,500
• If you are covering your spouse, your spouse will also have the option to participate in the HSA incentive plan.

We find that these effects are sustainable over time and clinically meaningful. This result is of critical importance, as it confirms that workplace wellness programs can help contain the current epidemic of lifestyle---related diseases, the main driver of premature morbidity and mortality in the United States.”

Employee privacy and confidentiality remains extremely important to DePauw University and the Office of Human Resources.
Employers, like DePauw, that sponsor one or more self-insured, HIPPA-covered group health plans-group health, dental, vision, pharmacy benefits, long-term care, health care reimbursement spending accounts, or employee assistance programs-are required to comply with all relevant Health Insurance Portability and Accountability Act of 1996 (HIPPA) regulations.
HIPAA includes provisions of Federal law governing health coverage portability, health information privacy, administrative simplification, medical savings account and long-term care insurance.

Once your personal profile is created, you will see a page with the following:
• How incentives are earned• Specific activities available to participate in• Incentive amounts and requirements for earning

Depending on the activity, data may be logged and captured by:
• Self-reporting the data• Syncing personal activity device (optional)• Employee Wellness Clinic

Wellness Incentive Earnings
Reasonable Accommodations will be offered to employees or their spouse with health conditions or disabilities that makes it medically inadvisable or unreasonably difficult to attempt to meet a standard in a wellness program.

Know Your Numbers
• Complete your annual physical and biometrics by November 30, 2019, to earn $250.
• Provide the DePauw Wellness Affidavit to your primary care physician at the time of your appointment.
• Physicals and biometrics obtained this year between July 1 to December 31 can begin uploading affidavits beginning December 1, 2018
• Forms will be made available online to access, print and take to your provider.
• To ensure privacy, employees and spouses will upload their form directly into the wellness portal.

Lifestyle Programs – Invest in Yourself
Lifestyle Program includes a variety of activities to choose from, which will be listed on the wellness portal and on the Human Resources Webpage.
Programs will be available via the wellness portal, the Employee Wellness Clinic, the Lilly Center, the Employee Assistance Program (EAP), or can be done on your own. Earned quarterly or annually, depending on program. Incentives range from $50 to $150, depending on the
completed activity. Certification of Completion is required for each activity
