What is Lighten Hospice Care?Lighten Hospice practices a Holis-tic Model when caring for terminal-ly-ill patients. We ensure that the true needs of each patient and their family are met by a team of com-passionate and caring professionals.

Lighten Hospice Care is a physician directed, RN coordinated program of care that responds to the physical, emotional, and spiritual needs of the terminally-ill patient and his/her family.

Lighten Hospice Care is offered in homes, as-sisted living and skilled nursing facilities, hospi-tals, or any other agency within our service area.

We go the extra mile in facilitating a peaceful passing with comfort, dig-nity, respect, compassion and love. Conventional medical procedures are coupled with complementary compassionate services, to holisti-

Lighten Hospice Care service area includes: Davis, Weber, Summit, Morgan, Box Elder, and Cache Counties.

Lighten provides holistic end-of-life health-care, offers support and guidance to the griev-ing, and provides information and education to our communities.

Lighten strives to assist patients in a journey of peaceful passing in all respects. Our goal is to empower patient’s final journey home with the utmost dignity and integrity.

Lighten does not discriminate on the basis of race, color, national origin, disability, diag-nosis, age, sex, religion, sexual orientation, or provisions of services and benefits with any-one they admit into their care.

Hospice care is paid for by: Medicare, Medicaid, or private insurance.

Medical Drector a physician from the community who works with the patient’s at-tending physician in providing the appropriate, qual-ity comfort care.

Prayer Medical Drector the power of prayer has been a true, timeless, doc-umented form of healing and in-ner peace. Respectfully individual-ized, it can yield truly amazing results.

Aromatherapy pure essential oils can have a favorable impact on patient comfort.

Massage/Reiki/Healing Touch used to restore and maintain wholeness through harmony and balance creat-ing relaxation and peace for the patient.

Reflexology/Acupressure di-rect trigger points release mus-cular tension, relieve stress, ease pain, and relax the mind and body.

Visualization/Guided Imagery used to stimulate a calming or healing re-sponse in the body, mind, and spirit.

Music therapeutic and soothing music is a creative, non-verbal, powerful tool for anyone. Music can bridge the silence of loneliness, soothe, ease pain, and tap into rich and sacred memories of life.

Art Therapy drawing, coloring, has been shown to allow expression of emotions further enhancing a letting go and peace.

Licensed Nurses trained to provide palliative care, monitor disease process, and man-age all symptoms for patient comfort.

Certified Hos-pice Aides to as-sist with personal care, home exer-cises, and activi-ties of daily living.

Spiritual Care a chaplain is on staff to meet the patient’s spiritual needs; and sup-port the patient’s own religious be-liefs, minister, or priest as necessary.

Volunteers to provide respite for family/care-givers, provide support and companionship.

Physical, Occu-pational, Speech Therapies, and Nutritional Coun-seling available to assess and provide
