Step 3: Final Step

• After filling the Form 6 Go to Submitted Applications > Form 6 Acknowledgement

• Click ‘Select’ link and Generate Acknowledgement button• Take a printout of the Acknowledgement and send it to the

address listed in the printed acknowledgement form with the copy of supporting documents

General Guidelines for Form 6

• You will be asked to upload Photo – Recent Passport Size Photograph (3.5CM X 3.5 CM) and Photo should be in .jpg Format and Maximum Size of Image should be 50KB only

• You will be asked to upload Age Proof - should be in .pdf Format and Maximum Size of PDF should be 1MB only

• You will be asked to upload address proof - should be in .pdf Format and Maximum Size of PDF should be 1MB only

General Guidelines for Form 6

• It might take few iterations to fill the Form 6 – So have patience

• You can fill Form 6 for your spouse – Your spouse can use the same address proof as yours

• Submit the hardcopy of Form 6 and self attested supporting documents(Age and Address) along with photo(optional) in person or by speed/registered post

Acceptable Address Proofs

• Bank / Kisan / Post Office current Pass Book, or• Applicant’s Ration Card / Passport /Adhaar Card / Driving

license / Income Tax Return filed or Assessment Order, or• Latest Water/ Telephone /post paid mobile bill/ Electricity /

Gas Connection Bill(This is the document you got when you took the gas connection and not the monthly Gas Refill bill) for that address, either in the name of the applicant or that of his / her immediate relation like parents etc. or

• rent or tenancy agreement or• Postal department’s post received / delivered in the

applicant’s name at the given address.• An affidavit stating the present address.

Acceptable Age Proof• a) Birth certificate issued by a Municipal Authority or district office of the

Registrar of Births & Deaths or Baptism certificate; orb) Birth certificate from the school (Govt./Recognized) last attended by the applicant or any other recognized educational institution; orc) If a person is class 10 or more pass, he should give a copy of the mark sheet of class 10 or 12, if it contains date of birth as a proof of date of birth.d) Mark sheet of class 8 if it contains date of birth; ore) Mark sheet of class 5 if it contains date of birth; orf) A declaration in prescribed format given in Annexure-1 made by either of his parents if the person is not educated till class 10 (in those cases where parent himself for verification before BLO/ERO/AERO); org) A certificate of his age given by a sarpanch of the concerned Gram Panchayat or by a member of the concerned Municipal Corporation / Municipal Committee, the person is not educated till class 10 and both the parents are not alive.h) Any other Government document showing age e.g. Passport.
