What’s NewNEW!!! Medical coverage is now available to

part-time associates! For 2010, part-time associ-

ates can choose medical coverage through the

TakeCare $10,000 Max medical plan.

Medical Enrollment Must-KnowsYou must go to the Your Benefits Resources Web

site and actively enroll in the new medical plan by

November 13 to have medical coverage in 2010! If

you don’t actively enroll, you won’t have medical

coverage next year.

Your Medical Option for 2010The TakeCare $10,000 Max Plan• The TakeCare $10,000 Max medical plan

is available to you and your eligible dependents.

• You must use a TakeCare participating provider.

The TakeCare network is the largest on-island

network of medical providers and pharmacies,

including the FHP clinic. No medical benefits are

payable for services received from a non-network

provider. To find a TakeCare participating provider,

go to www.takecareasia.com or call the

Customer Service department on Guam at

1-671-647-3526.

• The TakeCare $10,000 Max plan has benefit limits

on some services, such as primary office visits.

When you have reached the limit for a plan serv-

ice, the plan will not pay any more benefits for that

service. For example, the plan has a $200 limit

(per covered individual per calendar year) on pri-

mary office visits. This means that after you or a

covered dependent has incurred $200 in primary

office visit expenses in a calendar year, the plan

will not pay any more for primary office visit

expenses for that person.

For information on your medical coverage and the

plan limits, see the Medical Plan chapter of the

Benefits Summary for Guam part-time associates.

To verify that you and your dependents are eligible for

coverage in 2010, see the Eligibility and Enrollment

chapter of the Benefits Summary. As part of the ongo-

ing dependent verification process, the Company may

ask you to provide documentation of eligibility such as

a marriage license or birth certificate.

What’s New for 2010• New online beneficiary designation

Term Life Insurance Enrollment Must-Knows • You must designate the beneficiary(ies) for

your life insurance coverage during your

Annual Enrollment session using the new

online Beneficiary Form! This online designation

will replace any prior designation you have made

on paper forms.

• You can enroll in life insurance during Annual

Enrollment or if you experience a qualified status

change—by calling the Benefits Choice Center.

• If you are currently enrolled in life insurance, you

do not have to reenroll during Annual

Enrollment—you will keep your coverage for 2010

unless you make a change.

Your 2010 Term Life Insurance OptionsTerm Life Insurance for Part-Time AssociatesYou can buy life insurance coverage of $20,000

for yourself. If your death is the result of a covered

accident, the plan pays your beneficiary an

additional $20,000.

Term Life Insurance for Your Spouse and Dependent ChildrenYou also can purchase Dependent Term Life Insurance

for your spouse or same-sex domestic partner in the

amount of $2,500 and/or your dependent children in

the amount of $2,500 for each eligible child age six

months and older ($500 for a child under six months).

To find out more about term life insurance coverage, see

the Life Insurance chapter of the Benefits Summary.

To verify that you and your dependents are eligible

for coverage in 2010, see the Eligibility and

Enrollment chapter of the Benefits Summary.

You MUST Designate Your Life Insurance

Beneficiaries Online during Annual Enrollment!

When you reach the “Enroll in Your Benefits” page

during your enrollment session, you’ll see a link

“Choose a Beneficiary.” You must complete an

online beneficiary designation if you have life

insurance coverage.

You’ll need the following information for each benefi-

ciary you designate: your beneficiary’s date of birth.

The online beneficiary designations you make will

replace any prior designations you have made on

paper forms.

Term Life Insurance for Part-time Hourly Associates and Their Families

What’s New for 2010There are no changes to your disability plan or your

disability payroll deductions in 2010.

Disability Coverage Enrollment Must-Knows• If you opted out of disability coverage when you

were hired, you can enroll in disability coverage

during Annual Enrollment or if you experience a

qualified status change—by calling the Benefits

Choice Center.

• If you are currently enrolled in disability, you do

not have to reenroll during Annual Enrollment—

you will keep your coverage for 2010 unless you

make a change.

Your 2010 Disability CoverageIf an illness or injury leaves you unable to work, your

bills keep coming even though your paychecks stop.

When you are covered under the short-term disabili-

ty plan, you’ll continue to receive part of your income

if a qualified disability keeps you at home. That’s

why disability coverage is such an important finan-

cial safety net for you and your family.

The short-term disability plan begins paying 50% of

your covered weekly base pay (up to $125 per

week) after an illness or injury has kept you unable

to work for 14 consecutive calendar days. Short-

term disability benefits continue for an approved

period of disability up to 26 weeks (your 14-day

elimination period plus your 24 weeks of coverage).

Short-term disability benefits are reduced by other

income you receive (such as Social Security and

Workers’ Compensation) while you are disabled.

For more information on how the disability plan

works, see the The Short-term Disability Plan

chapter in the Benefits Summary.
