Click on “Suppliers”
Click on “Get details on how to become a supplier”
Click on “Complete the vendor registration form”
The below window will appear, click on “open”
Fill out the vendor registration form
Vendor registration form tips
• You may fill out the form by placing your cursor in the form and filling in the blanks.
• You may also print off the form and hand write the information
When complete, click on “Print”
Submitting the registration form Make sure your registration form is complete and all required forms (W-9’s, certificates, etc.) are included in your submittal. You may submit your application package to:

What happens next?
• You can expect an email acknowledgment that your registration form was received within two weeks of submittal.
• As long as all required information has been submitted, a final decision on your vendor registration can be expected within 90 days of submittal.
