Patients with an ACL deficient knee are having an anterolateral instability that causes “giving way” of the knee.

Trying to “de-rotate” and control instability of the ACL deficient knee in a direct way with a brace is very hard to do.

Therefore the DJO™ ligament braces try to stabilise the knee in an indirect way through the biomechanical 4-point design.

The 4-point system is clinically proven to be the most effective way to prevent anterior translation of the tibia.

By effectively reducing or preventing (*) anterior translation the 4-point brace in an indirect way avoids or reduces the antero-lateral instability (endorotation) in ACL defi-cient knees and allows the patients to regain knee stability.

Aggressive eccentric contractions of the quadriceps produce significant anterior tibial translation and can injure the ACL or permanently stretch out an ACL graft (DeMorat et al., AJSM, 2004). Additional studies show quadricep-intensive activities should be avoided at joint angles, particularly the last 30 degrees of extension, when strains are at a maximum. DJO™ has developed a tool to address maximising ACL and ACL graft protection in these “at risk” positions.

Small knee flexion angles at initial contact during landing tasks may contribute to ACL injuries. The FourcePoint™ Hinge can be used to control joint range-of-motion by increasing the knee flexion angle during landing to result in a decrease of anterior shear force on the ACL by 9-13% (Garret and Yu, AJSM, 2004).

The DonJoy® FourcePoint™ utilises a spring mechanism in the hinge to apply gradually increasing resistance during knee extension. The resistance engages in the last 25 degrees of extension (relative to the extension stop).

Three adjustment settings on the hinge allow for varying the amount of resistance to meet the specifications of the physician’s protocols and the patient’s comfort. The hinge may also be turned “off” for no resistance or “on” for resistance.

The resistance serves three critical roles: • First, it reduces the time spent near full extension or in the “at risk” position.• Second, it increases the posterior load applied by the brace on the tibia to prevent anterior tibial translation.• Third, it eliminates the extension shock felt when a patient extends into a 10 degree standard rigid stop.

FourcePoint™ Hinge Technology works to enhance DonJoy’s Four-Points-of-Leverage™ design by damping knee joint extension, which improves the mechanical performance of the brace and reduces shear forces at the knee. Addressing the “at-risk” position, the hinge resistance engages in the last 25 degrees of knee extension.

The FourcePoint™ hinge combined with the Four-Points-of-Leverage™ cuff and strapping design provides a more comfortable brace that reduces anterior shear forces at the knee. Decreasing anterior shear forces at the knee are beneficial for patients by reducing strain on the healing ACL graft, providing stability to an unstable, ACL deficient knee or for prophylactic use.

DESCrIPtIOnPolycentric hinge motion stabilises the knee while matching natural knee kinematics. Polycentric hinges also maximise the anterior/poste-rior and varus/valgus stabilising forces of the brace.

DESCrIPtIOnThis patented hinge design incor-porates a spring leaf and damp-ening mechanism that provides increasing resistance in the last 25º of extension to reduce exten-sion shock and increase posterior tibial loads. The FourcePoint™ hinge is shipped standard with a 10º extension stop to engage resistance at 25º.

DESCrIPtIOnThe DropLock polycentric hinge is a spring-loaded system which automatically engages the lock-ing device when the patient fully extends their leg. The DropLock hinge has an easy push button release to unlock, allowing free ROM for assisted gait. There are no additional Flexion or Extension stops available with DropLock Hinge design.

DESCrIPtIOnThis polyaxial hinge comes with a telescoping condyle pad that allows patient-controlled addi-tional unloading on the base load of 1 to 7° built into the frame. A floam condyle pad is included for increased patient comfort and compliance.

Patient registry data was collected from December 1994 to December 1998. At the time of database closure, we expected follow-up on 4100 patients and received follow-up on 2370 patients (57.8%). Physician diagnosed healing determined patient outcome in the patient registry. All patients were treated for 30 minutes per day, and devices were programmed to provide a maximum of 270 days of treatment. 10 The success rate of 5184 patients in pre-marketing clinical data was 60.7% and was maintained at 2 years post treatment with 90% follow-up of all healed fractures. In the pre-market study non-union was considered to be established when a minimum of nine months had elapsed since injury and the fracture site showed no visibly progressive signs of healing for a minimum of 3 months. Patient success was defined as three out of four corticies bridged on radiographic and no pain or motion at the fracture site. INDICATION: Noninvasive treatment of an established nonunion acquired secondary to trauma, excluding vertebrae and all flat bones. A nonunion is considered to be established when the fracture site shows no visibly progressive signs of healing.

CONTRAINDICATIONS: Use of this device is contraindicated in individuals having a synovial pseudarthrosis.Demand-type pacemaker or implantable cardiovertor defibrillator (ICD) operation may be adversely affected by exposure to magnetic fields. Physicians should not prescribe the DonJoy OL1000 for applications that may place the treatment transducers in close proximity to the pacemaker. Further screening by the attending cardiologist is recommended (such as with an electrocardiogram). The DonJoy OL1000 should not be used in the presence of external or internal fixation devices that are constructed from magnetic materials. (NOTE: Almost all fracture fixation devices implanted today are made from non-magnetic materials.)

WARNINGS: The safety and effectiveness of the use of this device on individuals lacking skeletal maturity have not been established. Animal studies conducted to date do not suggest any long-term significant adverse effects from use of this device. However, long-term effects in humans are unknown. Teratological studies have not been performed with this device. The safety of use of this device during pregnancy or nursing in humans has not been established.

PRECAUTIONS: Weight bearing is not advised in the presence of extreme motion at the nonunion site. In the presence of a malaligned nonunion, careful consideration of the use of this device must be undertaken on an individual basis, as treatment with this device is not intended to alter or affect the degree of malalignment. The safety and effectiveness of the use of this device on individuals with nonunion secondary to, or in conjunction with, a pathological condition have not been established. This device should not be used if there are mental or physical conditions that preclude patient compliance with the physician and device instructions. When conditions of atrophy are present or when fractures have remained unhealed for long periods of time, there may be less successful results.
