Welcome to Anthem Blue Cross and Blue Shield (Anthem) As an Anthem member, you must choose a primary medical provider (PMP). This directory lists Anthem providers, including hospitals, pharmacies, PMPs, specialists and providers first by county and second by provider type. It also lists PMPs by last name and by the languages they speak. Please read your member handbook. It tells you how your Anthem health plan works. It also tells you which services are covered and which ones are not covered.

For each doctor listed, you will find: 1. The doctors specialty. 2. The hours for the office, clinic, center or group. x You may find a symbol () next to the PMP name. This means the doctor is not taking

new patients at this time. x You may find a symbol () next to the PMP name. This means the doctor is board-

certified. x You may find a symbol () next to the PMP name. This means the doctor has ADA

accessibility wheelchairs can be used. x You may find a symbol (Y) next to the PMP name. This means the office has access

along public transportation routes. x You may find a symbol () next to the PMP name. This means the doctor is a Hoosier

Healthwise and Hoosier Care Connect provider. x You may find a symbol (z) next to the PMP name. This means the doctor is a Healthy

Indiana Plan provider. 3. The hospital where the doctor can send you for care.

Call the Customer Care Center if: x You want more details about a doctor listed in the directory. x You are a patient of a doctor who is not taking new patients, but who will keep you as a

patient. x The doctor you have now is not listed. (You may need to choose a new doctor.) x You want to know if a provider is still with Anthem.

Pharmacy benefits

We cover a wide range of prescriptions and many over-the-counter medicines. HHW members and HIP Maternity members get their pharmacy benefits from Indiana Medicaid through a company called Catamaran. HIP (including HIP State Plan) and Hoosier Care Connect members get their pharmacy benefits from Anthem through a company called Express Scripts.
