DESCRIPTION

During routine maintenance, the engineer found the

grinding machine in workshop with abnormal

vibration. After checking, he found electric motor port

side ball bearing was not in good condition.

Overhauled the electric motor and renewed the ball

bearing.

IMMEDIATE / CORRECTIVE ACTION

Stop using the Electric Motor and Overhaul for change

SUPERINTENDENT COMMENT

Safety officer to take periodical inspection during his

routine safety visit and assess the condition of the

grinder whether safe to use. Defective tools or

equipment should not be used unless rectified. As the

grinder is high rpm and which may endanger the

human serious injury special safety precautions to take

as per company procedure to avoid same situation.

Appreciate your immediate action to rectify the same.

SAFETY MESSAGES

HSP-09 – General Safety. In view of the danger to health, which may arise from prolonged contact with oil, personal hygiene is most important. Direct skin contact with oil or with oily clothing should be avoided.

“We have placed an emphasis on the need for safety, and a training department has been set up to cater to this requirement. Our training superintendents have customized shipboard trainings to cater to our specific

needs and have religiously gone on board to implement them. DESCRIPTION

The Master of the Precedence noticed that hot work at the

SBOT Jetty shore cargo pipeline repair job was still in progress

when the vessel berthed at 0400LT on 29-06-18. The worksite

was just about 100 m off the berth and this is high risk

operation for our vessel to connect the shore loading arm and

discharge her cargo. He informed the charterer to contact the

terminal and to confirm when they can complete the hot work

IMMEDIATE / CORRECTIVE ACTION

The master does not agree that is safe for both terminal and

vessel if commence the discharge of any cargo under this

condition.

DIRECTOR COMMENT

Your action had possibly prevented a worst case scenario of an

explosion that could result in several severe fatalities, huge

financial and environmental damages. On behalf of the

management, I thank you for your prompt action this morning

to STOP cargo operation in view of the unsafe condition taking

place near to vessel’s position and possibly on/in way of the

shore cargo line.

OPERATIONS

SIRE INSPECTIONS – GENERAL VIEW

Keeping the laundry room clean and tidy

Segregating working & daily use clothes

High risk (fire hazard) – handing clothes

around lightings

The SIRE system is a very large database of up-to-date information about tankers and barges. Essentially, SIRE has

focused tanker industry awareness on the importance of meeting satisfactory tanker quality and ship safety

standards.

“Each time a ship is nominated to a charterer and considered to lift cargo at a terminal which requires the consent

of an oil major, the charterer will refer the nomination to the oil major vetting department. The oil major will then

‘vet’ the ship. This may involve the oil major inspecting the ship. If so, the inspector will usually complete a Vessel

Inspection Questionnaire which is uploaded into the Ship Inspection Report program (SIRE) System. If no inspection is

required, the oil major may review previous SIRE reports. Owners must also provide and maintain a Vessel

Particulars Questionnaire.”

A SIRE inspection that reflects the efforts that are made by management and vessel personnel to operate the vessel

safely, efficiently.

Overall safety during SIRE inspections

Ensure full PPE is worn at all times when outside accommodation or in machinery spaces, no rank

exemption!! (To be comply with company PPE Matrix at all times)

If H2S or Benzene is present in the cargo, make this known to the inspector immediately on boarding and

ensure the watchman is aware of this.

Ensure ISGOTT Chapter 10 and OCIMF guidance on entering enclosed spaces is followed TO THE LETTER.

If any unusual situation exists, ensure that this is fully explained PRIOR to the commencement of the

inspection.

Boarding arrangement to be ready and checked by safety officer. (Chief Officer)

Ensure crew members are full aware of inspection criteria, well trained and know the procedures to be follow

up as per policy and standards.

The gangway watchman challenges the inspector once boarded:

Ask for SIRE ID

Ask the inspector to sign a Visitor’s Book

Provide the inspector with an Emergency Stations Card and ensure the watchman knows the requirements

Ask to see inside the inspector’s bag

Check that inspector’s mobile phone is switched Off

Keep in mind at all time: Everything within the vessel must be checked for their operational integrity rendering

officers and the crew to toil endlessly to ensure everything in place. Even if checks are carried out regularly, the

time before vetting calls for everything to be rechecked and verified.
