Accessing ActivHACC servicesService provisions are based on client eligibility and prioritisation of needs and eligibility is assessed by the WA Assessment Framework. For information about other supports available, please contact Commonwealth Respite and Carelink Centres on 1800 052 222. For referral to regional Activ HACC Services, please contact Client Services on 1800 622 351. For further information about Activ HACC Programs, please contact your local office. If you do not speak English, you can communicate with us through a 24 hours telephone interpreter service by calling 131 450 from within Australia.

We provide a range of opportunities and choices for people with disabilities and their families to pursue a better life, living and working in the community to their fullest.

Home and Community Care (HACC) ServicesActiv’s HACC Services are funded by the HACC Program, a joint Commonwealth and State funded program administered by the WA Department of Health.

We support people with disabilities and their carers to build and maintain the skills needed to carry out everyday activities and continue living independently in the community.

Services are delivered by trained and experienced staff with police clearances.Services are provided across the metropolitan area and in country regions of South West WA, Albany, Geraldton and Esperance.

Services delivered are:Social SupportOne on one and small group assistance, addressing each person’s need for social contact or accompaniment to be able to participate in community life.

Centre Based Day CareStructured group activities at an Activ HACC centre, including evening and weekend activities.Transport to the centre may be available.

Respite CareAssistance to carers so they may have relief from their caring role and pursue other activities or interests.

TransportTransportation in an Activ service vehicle, staff member’s vehicle or indirectly through taxi voucher or subsidies.

Cost of servicesActiv’s HACC Services fees are in line with WA HACC Fees Policy.
