Mission
1. Inform the community about prevalent types of cancer and diseases that affect the Hmong population and the importance of getting screened.
2. Provide an environment for pre-health undergraduates to obtain hands-on clinical experience.
3. Gather quantitative and qualitative data to assess the health status of the Hmong community.
Goals SHORT TERM OBJECTIVES:
● Recruit interns who are dedicated to HLUB Clinic’s mission.
● Promote cancer awareness through outreaching and educational presentations.
● Provide medical services and patient navigation assistance.
Goals continued...LONG TERM OBJECTIVES
● Reduce the cancer-related disparities in the Hmong community.
● Ensure that all Hmong individuals obtain skills and knowledge to maintain a healthy lifestyle.
● Participate in the planning and implementation of research activities designed to identify the health care needs of the Hmong community.
● Network with other organizations to achieve common objectives.

Patient Follow Up Procedures
● If patient has insurance, we will refer them to their primary care doctor for follow up care. ○ A letter and the patient’s chart will be given to the
patient to give to their PCP.○ Intern will follow up with patient to make sure patient
has been seen with their PCP.● If patient does not have insurance, patients will get
assistance on getting insurance through Covered California.

A free medical service to the Sacramento community HLUB Healthcare
Intern requirements● 1 Year commitment
○ Mandatory general meetings every month on the 1st Friday○ Mandatory clinic events every month on the 2nd Sunday○ Mandatory training, clinic shadowing○ Must go through ICC and obtain badge○ Be able to provide own transportation since most events take place in Sac○ Must pass clinic test with 75% or better○ Must fulfill outreach and fundraising requirements○ Must join one health committee and one supporting committee

● 1st recruitment term: ○ Winter 16 -End of Fall 17
● 2nd recruitment term:○ Summer 17- End of Spring 18
Benefits
● Receive transcript notation and can add IMD194 course (up to 3 units) P/NP
● Receive letter recommendation from medical director/preceptor
● Gain experience in a healthcare setting and patient interaction
● Work closely with the hmong community in Sacramento● But, don’t forget:
