The Health Insurance Portability and Accountability Act (HIPAA) was adopted in 1996 to protect the privacy and security of individually identifiable or protected health information (PHI). It also gives individuals rights over their health information.

The Privacy Rule set standards for the protection of health information for three types of covered entities: health plans, health care clearinghouses and health care providers who conduct standard health care transactions electronically.

The Security Rule sets up safeguards that covered entities and their business associates must implement to protect the confidentiality, integrity, and availability of electronic protected health information (ePHI).

The Breach Notification Rule requires covered entities to notify affected individuals and the U.S. Department of Health & Human Services (HHS) in the event of a breach of unsecured PHI.
