Task 2.3: Bundle Daily Review

** To expand Bundle Daily Review Section, Click Add:

The bundle daily review section should be filled:

1. At Bundle Start Date

2. At least once per week during ICU stay.

3. At Device removal date

4. At End of Surveillance date (Device removal date + 2 Calendar

days)

5. At Development of symptoms date

6. At Any significant change in the patient status.

** Date of first review:

1. Either the first date of admission to ICU if pt was admitted with

this device

2. Or the date of insertion if the device was inserted in this ICU on

this admission occasion to this ICU.

Summary

On the 17th, Ahmad central line was removed and he was transferred to general ward.

On 18th , Ahmad started having fever and chills and 2 blood cultures were taken on 18th and 19th.

Staph. epidermidis was isolated in both cultures.

Create new encounter with Encounter Type: Admission /Transfer to Units and Encounter reason name of the Non ICU Unit (General Ward ) he was transferred to (date of encounter 17th)

Daily bundle monitoring for 2 CD days (17th and 18th) continues , date of device removal being CD 1, focus only on BSI signs, symptoms and positive blood culture.

Fill the CLABSI event form (18th) as signs and symptoms are there.

After 2 CD days (on the 19th) , surveillance is stopped.

Continue daily bundle review for Foley's on 17th and 18th . Stop on 19th.

Create new Encounter: End of Surveillance and

Reason: Moved to Non Surveillance Unit +2CD(date 17th and 18th)

On the 12th, asecond central line was inserted and the first oneremoved due to blockage.

Encounter Type Central Line –Bundle –Event and Encounter Reason Central Line Insertion (UDF: Insertion, date and location only and Central Line Bundle Form) for the 1ST central line.

Encounter Type Urinary Catheter-Bundle-Event and Encounter Reason Urinary Catheter Insertion (UDF: Insertion, date and location only, and Urinary Catheter Bundle Form)

Encounter Type VAP-Bundle Event and Encounter Reason Ventilator Insertion (UDF: date and location only, Ventilator Bundle Form)

On the 12th : Update the Bundle form of the first central line with removal information

On 12th, Open new UDF for the 2nd central line and fill insertion as well as daily bundle review.

Continue daily monitoring on the new Bundle form

Scenario 11Transfer Out of NICU with a Central Line

Summary

Ventilator was removed on 9th March. Baby of Sarah remained for two weeks in NICU. During that time (20th March), he develops a CLABSI event confirmed by blood culture. On 23rd

March, he is transferred out of NICU with the central line in situ.

Continue daily monitoring on the Central Line Bundle form for 2 calendar days’ 23rd and 24th

After 2 days, create new encounter with Encounter Type End of Surveillance and Encounter Reason Moved to Non Surveillance Unit +2CD (date March 25th)

Scenario 13Readmission to Surveillance Unit

Summary

MUHAIB is a 21 years old male who was admitted on the 15th of March to ICU and 3 devices were inserted (Central Line, Ventilator inside ICU, and Urinary Catheter outside ICU). The ventilator was disconnected on the 20th of March. Patient was transferred to non-ICU unit with the central line and urinary catheter on the 29th of March.
