ATTACHMENT B: SERVICES AND PRICING
Services and Pricing Agreement for:
Minnesota Multistate Contracting Alliance for Pharmacy (MMCAP)
Minnesota Department of Administration

The Services and Pricing agreement shall remain in effect through the term of the
Contract including any extensions to the Contract. This Services and Pricing Agreement
supersedes any and all Services and Pricing agreements currently written by both parties
and is valid only with the Products sold through Contract MMS14034. Rates are subject
to an annual review and rates could change accordingly.
To Schedule a Regular Service Call: Please call the Henry Schein Service Hub at 1-800-645-6594 option 2. Henry Schein’s
service hours are Monday through Friday, from 8:00am to 5:00pm and will be billed at
the following rates, excluding holidays*.
•Arrival Fees – $15.00
• Labor Rate - $75.00 for the Initial ½ hour, $37.50 each additional ¼ hour per
Equipment Service Technician
Additionally:
• Installation of equipment purchased through the Henry Schein equipment department
will include labor. Extra fittings and or parts will be charged accordingly, including
shipping and handling as needed.
• Disconnection/reconnection of equipment from one center to another will incur labor
charges, and any freight and/or moving charges.
Emergency Calls: Response Time. Emergency call response time will be determined by
the local center based on geography. All emergency calls will be responded to within the
pre-determined time or less from the time the customer calls. If the call is late in day, the
emergency call may be scheduled for the first call of the next business day. In all cases,
Henry Schein will contact the Dental Office to confirm the Estimated Time of Arrival.
Rates for these calls will be as listed below. Future additional offices will be evaluated on
a case by case basis.

Equipment Service Technician
Note: An Emergency Call is defined as equipment failure that interrupts the ability to
treat patients and will be assessed by our Technical Advisors in our Henry Schein
Service Hubs.
• No air to all treatment rooms.
• No suction to all treatment rooms.
• No functioning method of sterilization.
• Major water leak that cannot be terminated at valve shut off.
• No functioning x-ray units in all treatment rooms.
X-ray Calibrations: Are recommended once a year and will be charged at the
established labor rate.
Equipment Installations/Relocations:
A service call requiring additional time and/or advance notice such as
(Disconnect/Reconnect, Relocation, Pre-Installation Site Visit, Site Check for New
Office Set-Ups or New Equipment Purchase) will need to be scheduled. Please contact
your Special Markets Equipment Team and they will coordinate with the appropriate
local Henry Schein center. A minimum of 2 weeks’ notice is recommended for these
types of service calls.
Service Parts:
In the event that additional parts are required, they will be ordered and a follow-up
service call will be scheduled when the parts arrive. A $10.99 Shipping/Handling Fee
will be added to the service invoice for all parts used during repair. In emergency
situations, the parts will be shipped to the local Dental Office, Next Day Air 10:30 am
delivery, at the customer’s request and approval of additional expense. These emergency
shipments will be labeled appropriately and require an office staff signature. Upon
receipt of the part(s), please call 1-800-645-6594, option 2 to schedule an appointment
with the technician.
Primary Technician:
A Primary Technician will be assigned to each of your locations. The primary technician
will be responsible for the maintenance and repairs of assigned locations. In the event
that the primary technician is unavailable a secondary technician will be identified.
Rentals:
In the event of equipment failure, Henry Schein will provide the following rentals at rates
specified. These rentals are intended for short-term use only until the customer
equipment can be properly repaired or replaced by Henry Schein. Should the equipment
be deemed unrepairable and the customer decides to purchase a replacement product
from Henry Schein within 30 days, we will credit the rental fee charged for that product.
•Single Head Air Compressor $95.00 per week
• Single Motor Vacuum Pump $95.00 per week
• Autoclave $95.00 per week

Rentals may not be an exact duplicate and are to subject to availability.
Rentals will be billed weekly per device plus any applicable freight or delivery/pick up
charges (minimal ½ Hour Service Rate) until device is returned.
ProRepair:
Service calls to pick up and drop off repairs sent to ProRepair will be invoiced at the
contracted service rate (minimum ½ Hour Service Rate).
Equipment Warranty Service:
The customer will receive the benefits of any and all manufacturers’ parts warranties.
Shipping and Handling charges are not covered by this warranty. Parts warranties do not
include preventive maintenance and those items subject to normal wear and tear. Ex.
Sterilizer gaskets, light bulbs, filters, water bottles, saliva ejector & high volume suction
tips & levers, nitrous and oxygen rubber goods and tubing. All manufacturers’ warranties
are subject to the manufacturers’ policies and the customer is responsible for all periodic
maintenance as per the installation and service manual.
Extended Labor Warranty for Equipment: Henry Schein will provide a 12-month labor warranty for all new equipment purchases.

Our services are simple to use:
1. Request a Mailer Kit:
Access us through a ProRepair app from your I phone, Blackberry, Droid
Email us at www.ProRepair.com
Call our toll-free number, (800) 367-3674, and notify a customer service
representative
If an immediate repair is needed, notify customer service that you would like to arrange for a Same-
Day UPS Pickup (additional charges apply).
2. Complete Work Order:
Fill out the work order form attached or located inside the Mailer Kit.
3. Shipping Instructions:
Place your repairs inside the protective envelope or box.
Attach the ARS label and hand the envelope or box to your UPS driver.
Keep a copy of the tracking number for your convenience.
Note: Tabletop sterilizers, please contact us by email at www.ProRepair.com or call our toll-free
number to arrange for a pickup.
Our operations are equipped to handle a large percentage of the equipment received. In the event
something cannot be repaired in-house, we utilize manufacturers or certified third-party repair
operations.

3674 and inform a ProRepair customer service representative of your needs. Rentals are available for
many types of small equipment and will be shipped to you immediately (rental fees may apply).

repair.

Terms
All ProRepair in-house repair service provided to MMCAP State of Minnesota will be discounted by
15% off current retail pricing.
All ProRepair manufacturer repair services provided to MMCAP State of Minnesota will be
discounted by 10% off current retail pricing.
Your repair discount cannot be combined with any other offer/promotion. You may use a
coupon instead of your repair discount, however; a coupon can only be applied to a single
repair, whereas your corporate account discount is applied to an entire repair order.
Please provide ProRepair with a list of all account numbers in order to provide the respective
discounts. Please inform us of any additions and/or deletions of account numbers to stay current with
discounting.
ProRepair will provide MMCAP State of Minnesota shipping supplies for each repair service.
MMCAP State of Minnesota agrees:
to use ProRepair services exclusively for Products purchased through Contract MMS14034.
does not require an estimate approval prior to repair if estimates are under $300.

Services and Pricing Agreement Term:
This agreement will run concurrently with Contract No. MMS14034 including any
Contract extensions. Either party shall have the right to cancel this agreement at any
time. All terms, rates, and privileges allowed under this agreement will be void if the
account is not in current standing.
