HealthPartners Minnesota Senior Health Options (MSHO)

MSHO is a voluntary managed care program that covers health care and Elderly Waiver services for people who:

• are age 65 and over,

• have Medical Assistance, and

• have both Medicare Parts A and B.

There are no added costs for you to join this program. While a member, you will get your Medical Assistance and Medicare Parts A, B and D through the plan.

From Oct. 1 through Feb. 14, we take calls from 8 a.m. to 8 p.m., seven days a week. You’ll speak with a representative. From Feb. 15 to Sept. 30, call us 8 a.m. to 8 p.m. Monday through Friday to speak with a representative. On Saturdays, Sundays and Federal holidays, you can leave a message and we’ll get back to you within one business day.

If you choose to leave MSHO, you will be enrolled in an MSC+ for your Medical Assistance services.

Important Information Clinics

The clinic information printed in this listing was correct as of December 14, 2017. You need to choose a primary care clinic and dental clinic when you enroll. Your primary care clinic can provide most of the health care services you need and will help coordinate your care. Please confirm with Member Services that your clinic is still a provider with our plan before signing up. Use the following list of clinics or our online provider search at healthpartners. com/msho to choose one medical and one dental clinic. Write the 2, 3 or 4 digit clinic number of these clinics on your enrollment form.

Cost Sharing

You may be required to pay an amount toward some health care services. This is called cost sharing. Some examples of cost sharing are copays or deductibles for certain services. You are responsible to pay your cost sharing amount to your provider. When you enroll in this plan, you’ll get your member ID card and information on how to access materials such as your Member Handbook and Provider Directory.

Member Identification (ID) Cards and Member Packet

Members will get a health plan Member Identification (ID) Card. You must show this card whenever you get health care services, including when you get a prescription filled. When you enroll in this plan, you will get a member packet that includes your Member ID Card, Member Handbook, and Provider Directory. These documents give you more details about your plan.

Service Accessibility

If you need special access to get services from a provider, you can obtain information by calling Member Services at 952-967-7029 or 1-888-820-4285 (TTY/TDD 711). This could include, but is not limited to, access for services such as availability of flexible hours, wheelchair access, or parking lot access.

Service Authorization

Our approval is needed for some services to be covered. You must get the approval before you get certain services or before we pay for them. Members will get a Member Handbook with more information about which services require authorization.

Services

• Care Coordination: When you enroll in the MSHO plan, you are assigned a Care Coordinator. He or she is your advocate. Your Care Coordinator will work with you, your family and your health care team to give you the best services, treatments and care. Care Coordinators are licensed health care professionals who can help with medical appointments, prescription drugs and refills, dental care, transportation, meals, interpreters, chore and companion services and home safety. You can get more information on care coordination services by contacting Member Services.

• Chiropractic Care: You may make your own appointments to go to a chiropractor. You must go to a chiropractor in the HealthPartners Chiropractic Network. You can get more information on how to access chiropractic services by contacting Member Services.

• Dental: You can continue seeing your dentist as long as he or she participates in this plan. Use the dental clinic list provided or our online provider search at healthpartners.com/spp or call Member Services.

• Elderly Waiver Home and Community Based Services: If you are age 65 or over and need help to stay in your home, a program called Elderly Waiver (EW) can help. EW offers more services than regular Medical Assistance. Services may include visits by a nurse, home care services, homemaker services, chore services, and more. Your Care Coordinator can arrange these services for you. If you want more information, please contact Member Services.

• Elderly Waiver Obligation: If you get EW services and your income is over the income limit, you may have to pay a monthly amount. This is called the EW waiver obligation. Your county will tell you about any EW waiver obligation when your eligibility is determined.

• Eye Care: You can get information on eye care services by calling Member Services.

• Home Care: Home Care services include the following: · Skilled nursing

· Rehabilitation therapies (for example: speech, physical, respiratory, and occupational)

· Home health aide · Medical equipment and supplies · Personal Care Assistant (PCA) · Home Care Nursing (HCN) services

You can get information on Home Care services by contacting Member Services.

• Hospitals: Each medical clinic listed also lists a hospital that it works with. You will go to the hospital associated with your primary care clinic for most of your hospital care. You can get more information on hospital services by contacting Member Services.

• Interpreter Services: American Sign Language (ASL) and language interpreter services are available to help you get services. Oral interpretation can be given to you in your language. You may be able to get written information in your language. Contact your Care Coordinator or Member Services to arrange for an interpreter.

• Medical Equipment and Supplies: Medical Equipment and Supplies is any medical equipment you can use in the home to improve quality of living. It can also provide therapeutic benefits to people with certain medical conditions or illnesses. Examples are nebulizers, wheelchairs, crutches and many others. You can get more informatoin on how to access Medical Equipment and Supplies by calling Member Services.

• Mental Health: Mental health services include care and treatment for depression, anxiety or other mental health issues that interfere with your daily functioning. You can get more information on Mental Health services by contacting Member Services.

• Nursing Home and Skilled Nursing Home Care: We will cover a total of 180 days of nursing home care. If you need nursing home care for more than 180 days, the Minnesota Department of Human Services (DHS) will pay for your care. This applies unless you were already in the nursing home when you joined our plan. You can get more information on nursing home services by contacting Member Services.

• Open Access Services: For some sevices, you may choose to get care from any health care provider, clinic, hospital, pharmacy, or family planning agency even if it is not in our network. These are called “open access services” and they include the following: · Family planning · Diagnosis of infertility

· Testing and treatment of sexually transmitted diseases

· Testing for AIDS or HIV-related conditions You can get more information on open access services

by contacting Member Services.

• Pharmacy: Members must use our network of pharmacies for drugs covered by our health plan. A list of pharmacies will be in your member materials. Remember to have the following available when you call to refill your prescription: · HealthPartners Member Identification (ID) Card · Minnesota Health Care Programs card · Cards for any other health coverage you have, such

as Medicare Part D or private insurance

It is important to remember to show both your Member Identification (ID) card and your Minnesota Health Care Programs card when picking up your prescriptions. To see HealthPartners drug formulary, go to healthpartners.com/spp. If you are enrolled in Families and Children, MSC+ or SNBC and have Medicare, you must enroll in a Medicare Prescription Drug Program (Medicare Part D) to get most of your drugs. You can get more information on pharmacy services by contacting Member Services.

• Specialty Care: Specialty care describes services you can get from doctors or surgeons whose training focuses primarily in one specific field of medicine. Examples include neurology, cardiology, and oncology, among others. You do not need a referral to see most of the specialists in this plan. You can get information on how to access specialty care by going to healthpartners.com/spp or contacting Member Services. This information is also in the HealthPartners MSHO Provider Directory.

• Substance Use Disorder: Substance use disorder (SUD) services provide care and treatment for substance use disorder. This can include abuse of alcohol, narcotics, or other drugs. Call Member Services for information about Substance Use Disorder providers.

• Transition of Care: When you join HealthPartners we will help you continue to get necessary health care services that were authorized before you joined our plan. Some examples are the following: · Orthodontia · Mental health services · Substance use disorder services · Services for at-risk pregnancy · Prescriptions

If you are currently getting care from a provider who is not in our provider network, we will help you transition to a participating provider. If you would like more information please call Member Services.

• Transportation: We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home if another primary care clinic is available within 30 miles. Exceptions to this rule can be made in certain circumstances. If you need a ride and there isn’t a closer clinic in our network, please call RideCareSM. RideCare can help you arrange rides to your health care appointments at no charge to you. Call 952-883-7400 or 1-888-288-1439 (TTY 952-883-6060 or 1-800-443-0156). Members will get a Member Handbook with more information about transportation.

• Urgent/After Hours/Emergency Care: If you are unsure of what care you need and want to speak to a nurse, call CareLineSM at 612-339-3663, or 1-800-551-0859 or (TTY 952-883-5474). This service is available to you 24 hours a day, seven days a week. The nurse can help you understand the best place to get care, such as an emergency room, urgent care or clinic appointment. The nurse can also help in many cases with home treatment advice. In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room.

You can get care in several settings, including transitional care centers, nursing homes and assisted living facilities. You also have access to full home and hospice care. As your needs change, our teams are here to help.

Convenience Clinics

Do you have a sore throat? An earache? When you can’t get to your primary care clinic, use a convenience clinic instead. They treat a variety of health concerns and you don’t need an appointment.

• Convenience clinics are found in stores like Target, Cub Foods and CVS pharmacies

• Your primary care clinic may also have a walk-in option
