Achieving a Network of Integrated Community-Based Medical Homes OCHP is leading a MDC-wide comprehensive planning process to develop a roadmap that will become the guide for leveraging both additional capital and operating dollars to expand our health delivery system. The Goal is to turn Community Health Systems into full spectrum portals to comprehensive health and services.
Leveraging – $25M
in GOB for Primary Care Facilities capital funds to generate nearly 150,000 added primary care visits (over 3 years) from the County’s existing 7 Federally Qualified Health Centers (FQHCs)
Target Population: Low Income Adults, including
the Parents of Medicaid-Enrolled Children
The Uninsured: District 2 • About 92,882 residents in District 2 are estimated not
to have Health Insurance. That is 17% of Miami-Dade County’s uninsured– See Map2
• 28% of residents of District 2 are enrolled in Medicaid1,5
• Median Household Income5 – Median income for District 2 residents was estimated at $29,683, ranging from $18,506 (zip 33142) to $35,897 (zip 33169).

Sampling of Chronic Disease Indicators District 14:
• Diabetes – 8.5% (54) of the County’s deaths due to diabetes were residents of District 1..
• Breast Cancer – 8% of the County’s 309 deaths due to breast cancer were District 1 residents.
o Zip code 33056 accounted for 42% (10) of District 1’s deaths due to breast cancer.
• Cardiovascular Disease – 8% of the County’s 5,916 deaths due to cardiovascular disease were residents of District 1.
Emergency Room Visits from District 13: • Over 38,000 visits to Miami-Dade County Emergency Departments were made by District 1 in
2011.
• 5% of the total number of ER visits by Miami-Dade County residents was made by residents of District 1.
ER Diversion & Chronic Disease Management Dual initiatives utilizing evidence-based approaches will assist individuals seeking primary care in receiving an immediate appointment – while still in the ER – for a primary care visit in their neighborhood. For individuals living with chronic diseases, the initiative focuses electronic-enabled mechanisms for the identification of conditions, case management by licensed professionals, and guided treatment.
Health Statistics at Zip Code and District Levels OCHP has developed critical relationships with State and local data sources to build a comprehensive database of nationally-comparative health and demographic data, as well as, health utilization and health system infrastructure. OCHP is able to present this data in baseline, trend analysis, projection and GIS formats. Attached are some of the maps that have been produced detailing District 1:
• District 1 Close-up – Details the names and locations of each of the major primary care facilities and hospitals in District 1.
• Miami-Dade County Safety-Net & Primary Care Centers – Details the names and locations of each of the
County’s Federally Qualified Healthcare Centers and Jackson Health System Primary Care Centers, as well as, identifies the locations of all county hospitals. District 1 boundaries are outlined in red.
• Miami-Dade County Distributions of the Population and the Uninsured – Describes the areas of highest
to lowest concentrations of the population in all of Miami-Dade County, overlaid by the distribution of uninsured and specifically outlines Commission District 1.
• Miami-Dade County Distribution Below Poverty – Details the population density of individuals below the
federal poverty level by census tract and outlines Commission District 1 boundaries in red.
