Habilitation NotesTraining Agenda

Welcome to ITC Habilitation notes (HAH) training.

Here at ITC we are going green and making completing HAH notes faster, easier, and better than ever before! With DDReports and DDTracking completing HAH notes is as easy as clicking a button!

User Login

1. Once you have been assigned a client, met with the guardians, and established habilitation hours to work; prior to working your first shift, it is your responsibility to contact your direct supervisor the Member Care Coordinator. The Member Care Coordinator will send you an E-mail.

2. From your desktop computer open the E-mail and click the Link.

3. Write down your user name located at the top of the page.

4. Create a password and security question and click submit.

Timecard Entry

Immediately after your shift complete the following steps:

1. Go to www.ddreports.com.2. Log in.3. Click on timecard.4. Select correct pay period and client.5. Enter day, start time, and end time.6. Select correct service and click submit.7. Go to www.ddreports.com on your computer.8. Print out your timecard and get guardian

signature and initials.

Please note it is solely your responsibility to print out your timecard and get required signatures and initials by the 1st and 16th of EVERY month that you work. Failure to do so can lead to late payment.

E-Trials

E-trials are the trials for the habilitation outcomes that providers run when working habilitation with an individual. These MUST be completed at the end of every shift. Please note each outcome and trial must be run every single habilitation shift.

E-trials can be done on your computer at www.ddtracking.com or on your smart phone or tablet at www.ddreports.com.

Progress Summary

Progress summary is a summary of the progress that an individual has made on their goals.

Example: John Doe is struggling with wiping independently due to physical limitations and coordination. He has made significant progress with using utensils during mealtime. He struggles with fine motor skills, but has made significant progress.

Progress Summaries can only be done on a computer at www.ddtracking.com Progress Summaries are due once a month for every month worked.

Functional Objectives

Functional objectives are a combination of the following criteria:

• Habilitation outcomes• Teaching Strategies

Functional Outcomes can be accessed at www.ddtracking.com Once you are logged in selected the desired client, click the reports tab, click on progress, and then click on functional objectives.

Teaching Strategy

Teaching strategies are the way we run a habilitation outcome. It is the plan or guideline a provider uses in order to assist an individual with achieving mastery in a particular goal. Every provider should know the outcomes and teaching strategies of their client prior to starting their habilitation shift.

Due Dates

Timecards are due by the 1st and 16th of each month. All habilitation notes are due by the 2nd of each month at 5:00pm. It is expected that all providers record worked shift immediately following their shift. It is also expected that providers complete e-trials immediately following worked shift.

1. Go to www.ddtracking.com. 2. Log in.3. Click on the appropriate client in the left hand

corner.4. Select correct pay period and client.5. Enter day, start time, and end time.6. Select correct service and click submit.7. Go to www.ddreports.com on your computer.8. Now click the plus sign; this sign will now be a

minus sign with a drop down box.

9. You will now see each of the individual’s goals. Adjacent to each goal is a box (or boxes) labeled “trial”. These trial boxes each have the same post bubble codes in them: I (independent), VP (verbal prompt), PH (physical help), TH (total help), and R (Refused).

10.Click the appropriate bubbles.

11.Click the minus sign to collapse the box.

12.Refresh the page and notice the red thumbs down is now a green thumbs up.

HAH Notes on Phone/Tablet

1. Go to www.ddreports.com.

2. Log in.

3. Once logged in, select timecard.

4. Ensure time period is correct as well as member. Then select add claim.

5. Now select service provided for that shift respite (RSP) habilitation (HAH) attendant care (ATC) housekeeping (HSK) and so on.

6. Now select the day worked, and add the start and finish time of shift worked (Touch each section to make selection) (Press set to enter time). Once you have entered the day, start and finish time of your shift, press save.

Please notice the words e-trials needednext to the date that was entered. This will only occur when you have worked

habilitation. Note a drop down menu is now present. In this menu each habilitation outcome is present. As well as each trial for each outcome. Scroll up and down to see all outcomes. Scroll side to side to complete each trial. Notice in each trial are several bubbles to choose from (I) Independent (VP) Verbal Prompting (PH) Physical Help (TH) Total Help (R) Refused. Select appropriate bubble.

Scroll up to the top of the page. Notice the minus sign next to the date. Press the minus sign to close menu. Now refresh the browser. Notice it does not read “needs e-trials”. This means e-trials for this date are complete.

Frequently Asked Questions

Q: Why aren’t my e-trials being saved?A: Try refreshing your browser, after closing the drop menu it will still

show e-trials needed or a red thumbs down until the browser has been refreshed.

Q: How do I know my login name? A: It is displayed at the top of the password and security question link

sent to you prior to your first log.

Q: What do I do if I forget my password?A: If you have forgotten your password, click underneath forgot

password and follow the steps or contact your Member Care Coordinator for assistance.

Q: Can I add a future date to my time card that I have not worked yet?

A: Yes, but only if you are printing it out right before you leave to your last shift to be worked prior to timecard day.

Q: Is it okay to wait until the day before time card day to input my times in?

A: Yes, but you should be entering them in immediately following your shift. ITC is NOT responsible for FAILURE to put in all times worked resulting in non-payment.
