Does the applicant or any of the owners hold an ownership interest in any business entity

which currently owes the GBRAA any sum of money?

Is applicant purchasing a commercial operation or the assets of a commercial operation that

is currently doing business at the GBRAA? If the answer is YES, identify the commercial

operator

Is applicant purchasing a commercial operation or the assets of a commercial operation that

has conducted business at the GBRAA within the past five (5) years? If the answer is YES,

identify the commercial operator.

Have any of applicant’s employees, owners, partners, shareholders, or members been

affiliated with a commercial operator that is currently doing business at GBRAA, or has

done business at GBRAA within the past five (5) years? If the answer is YES, identify all.

Original of completed and signed Application and Agreement for a Business Permit.

Permit Fee ($120.00 per year from October 1 to September 30, or prorated at $10.00 per

month for each month remaining through September 30). This permit fee must be renewed

by October 1 of each year..

Original Certificate of Insurance naming the Gulfport-Biloxi Regional Airport Authority as

an additional named insured. All insurance shall be written by an insurance company

licensed by the State of Mississippi (Call MS Ins. Commissioner to verify\1-800-562-2957).

Copy of Certificate of Incorporation (from Secretary of State), if applicable

Copy of permit issued by the U.S. Department of Transportation (for inter-state

transportation only), if applicable. (N/A to taxis)

Copy of permit issued by the Mississippi Public Service Commission, if applicable. (N/A to

taxis)

Proof of purchase of apportioned tag (for inter-state transportation only) if applicable. (N/A

to taxis)

Copy of the HCMVH Commission Authorization

Copy of City privilege License, if applicable

Copy of Mississippi Drivers License

Copy of published passenger rates (per Chapter 4, Section 11 – Page 33) for on demand

transportation (taxicabs, sedans, limousines, and vans) applications.

List of all vehicles for each category of ground transportation, including make, model, year,

color, vehicle number (if used) and how many passengers each vehicle will hold.

Inspection completed by an Airport Authority Operations Supervisor.
