Database Addiction took place at Lorraine Hewitt House which is situated in the heart of Brixton, it is staffed by busy, engaged, dedicated drugs workers, clinical psychologists, social workers, psychiatrists and adminis- trators including reception staff and all have a high work load. The care-centre sits at the nexus of several databases National Drug Treatment Monitoring System (NDTMS), Electronic Patient Journey System(EPJS), NHS Spine and others. How do databases act in Lorraine Hewitt House. All the work at the centre is interrelated to collections of information, that in turn evidence decisions, events and result in quantifiable structures that reciprocally create other in- dividual actions. This table is an attempt to think through the structural cou- pling of databases and the intention of the care-centre. Databases are more often then not regarded as a technical solution with scant regard is given to their culturalisation, sociality or the way the knowledge/power, or architectural relations are altered by them. The enterprise of the care-centre in it's own words is to “support anyone over the age of 18 with alcohol or drug problems who lives in Lambeth. Pro- viding a key worker to a service-user working with them to formulate a care plan for a journey through other services.” Most people work with the Elec- tronic Patient Journey System which has background processes, sorting algorithms that extract information into other systems such as the National Drug Treatment Monitoring System (NDTMS) whose enterprise is summa- rised as: 'process information from and for those involved in and around the governance of drug treatment.' The databases investigated are a form of list architecture that produce an abstraction of the care-centre modalities. They afford methods by which the materiality of the care-centres work can be managed and governed at multiple scales from with computation. This process of abstraction while al- lowing distance and overview from the gritty work of dealing with addiction also by necessity is always incomplete and structurally flawed representa- tion of the experience of the care-centre. The structure of the The National Drug Treatment Monitoring Service un- locks what is articulated about addiction from an idealogical, technical, po- litical, bureaucratic or governmental viewpoint which is used amongst other things for monetising addiction services. The NDTMS can be seen as pointers to a fluid formation of power that is a strategy in action. While this formulation can be useful in understanding this strategy as a navigable fluid architecture it tells us very little about how individuals manipulate/re- sist the modalities it creates. The logics represented on this table interrelate through disciplining the or- ganization and forcing compliance with various databases through such mechanisms as The Treatment Outcomes Profile (TOP) which purportedly “measures change and progress in key areas of the lives of people being treated in your drug and alcohol services.” This form of compliance affects overall funding, policing staff efficiency and job security. -------------------------------------------------------------------------------------------------- Table of information: The content in the centre of the tables is derived from documented conver- sations with drugs workers, clinical psychologists, social workers, and ad- ministrators including reception staff in the spring of 2015 and from email correspondence with representatives of Public Health England about the inner workings of the DAMS and NDTMS databases. The content around the edge of the tables is derived from text files (see round tables) that we collected throughout our research. We analysed the files to find the collocations of words in the documents creating an aesthet- ic discourse analysis to explore how is our language is informed by the ma- chines that process it? Each word in the list is related both to the one's below it and those that are adjacent to it. Comment 1: “I’ve got concerns; confidentiality is a thing of the past. EPJS used to be only in SLAM but now partners have access I believe there may be work/connections with the home office, police, etc.”

Comment 2: “EPJS is important because it’s multidisciplinary (across psychology, substance, prescriptions etc), but there may be some connections that are more about number-crunching, e.g. NDTMS (for example, the TOPS input section has questions about crime).”

Comment 3: “Questions for the database can be that they may cause distress to service-users. Therefore, some answers may be vague or inaccurate as a result of feeling our way through the moment of filling these in with service users.”

Comment 4: “Some entries are legal requirement. Others are outcome monitoring[?]. The PHQ9 form relates to recording depression within a treatment pathway. The GAD7 form relates to anxiety. These aspects of patients’ treatment pathway may be recorded and pursued based on recommendations of key workers and psychologists. Sometimes there is no clear course for psycho-logical intervention in treatment pathway.”

Comment 5: “In some respect, by choosing what forms and routes of treatment need to be undertaken, it’s possible that workers may

take recommendations for changes to managerial meetings.”

Where Does The Data Go?(Conversations With Public Health England)

Within the care-centre data entry and clinical tools have daily collections. At the end of the month submissions (of CSV) are collected by the DAMS system. At this moment data validation occurs, checked to make sure staff have appropriately filled in the forms.

Data collection occurs at the end of each month with the DAMS system opens for CSV submissions in the middle of the month. Data must be submitted to National Data Set (NDTMS Central) This is an 11 month cycle August to June. July is omitted in the production of Frozen Dataset which is for annual statistics. The data submitted is attributable to a service user by (initial, DOB, gender, only half the postcode) We found that DAMS uses both Role Based Permissions (people with certain roles/permissions can view things others cannot) and System Security Policies in which if you have entries in an area where by you have a 1 in 5 chance of people being identified through the data then that data must be suppressed. There is some provision made for pri-vate actors getting access to the DAMS?

Q. Do treatment providers only interact with the software and does the DAMS system comprise a different database system?

PHE: DAMS accepts CSV and exports CSV. It has a set of role

based permissions that determine the levels of visibility to differ-ent people. Treatment providers do have access to a local data-set, which can also be shared with other regional providers, which they can access. However I think it also adheres to the CSV extract, which renders data down to attributable only.

Q: In the data flow diagram the treatment providers and NTDMS regional teams deem to feedback to one – what are the particu-lars of that feedback (i.e. data cleaning, reports generation?)

PHE: The data flow diagram basically shows how the DAMS re-ports are generated and fed back to treatment providers on a monthly cycle.

Q: Do the regional teams extract different reports from DAMS than the NTA team and NDEC teams?

PHE: Yes. The latter two are responsible for generating reports for both treatment providers and a separate set of reports for management on monthly / quarterly basis. These reports are also FAO providers and partnerships. They also produce a Na-tional Treatment Journey File.

Q: Is a service user still identifiable in the DAMS database?

PHE: User attributable data is preserved.

Q: Is a service user still identifiable in the TOP & Modality CSV files extracted and imported into analysis software?

PHE: As this is derived from DAMS, the attributable data only factor still applies.

Q: Is the 'multiple analysis system' step writing to relational data-bases (labelled multiple SPSS datasets).

PHE: The time line view implies TOP reports being generated for an individual client.

Q: Are TOP reports generated bespoke to each user? If yes, are these the same TOP reports mentioned in right hand side of above diagram. Also I tried to get a sense of when pseudo an-onymisation of a record occurs from the extraction instructions in the CSV FILE FORMAT SPECIFICATION document. I couldn't find a specific answer – can you tell me where/when that happens and who is responsible for it?

PHE: Seems likely that this will be detailed in the DAMS docu-mentation.
