Player and Cheerleader Contract

THIS AGREEMENT is made between GRAHAM EAGLES YOUTH FOOTBALL & CHEERLEADER ORGANIZATION, and

_____________________________, the legal parent/guardian of ______________________________, a junior

player/cheerleader.

The Player/Cheerleader will:

1. Abide by the GEYF&C Organization rules, regulations and policies which include the Code of Conduct and the Coaches

Code of Ethics.

2. Abide by the Region’s rules, regulations, and policies; and

3. Pay all fees and costs associated with the Organization as described in the Graham Eagles Youth Football &

Cheerleaders Organization Agreement contract.

The Organization will:

1. Abide by the GEF&CY Organization, rules, regulations, and polices which include the Code of Conduct and the Coaches

Code of Ethics.

2. Abide by Region’s rules, regulations, and policies; and

3. Deliver the football education, training, and playing opportunities described in the Graham Eagles Youth Football &

Cheerleaders Organization Agreement contract.

I certify that I am the legal parent/guardian of player, ___________________________________, and have the authority to

agree and sign this contract and waiver on behalf of the player. I acknowledge that participation in this athletic activity can

result in serious injury or death to participants and agree to indemnify, defend and hold harmless the Graham Eagles Youth

Football and Cheerleaders Organization for any and all claims for harm or injuries suffered during participation in the

organization activities, including but not limited to play and practice. I further agree and to waive and release the Graham

Eagles Youth Football and Cheerleader Organization from any and all demands, claims, and actions, lawsuits, liability, and

damages of any kind arising from participation in the Organization activities. I agree that prior to filing any lawsuit against the

Organization, I must attempt to resolve my claim via mediation and arbitration in accordance with RCW 7.04A in Pierce Co.,

Washington.
