Roy SlackCementation Canada IncPresidentPresident elect of the CIM

roy Slack is a professional engineer with over 35 years of experience in mine construction, design, and development, on projects throughout Canada and around the world. In 1998 he started Cementation’s operations in north America as President. Cementation is a mine contracting and engineering firm that provides design-build and construction management services to the mining sector. Today Cementation is one of the largest mine contractors in north America and part of one of the largest mine contracting organizations in the world, the Cementation Group. In 2016 the company was recognized as a Gold Winner of Safest employer in Canada in the natural resources sector. roy is a graduate of Queen’s University in Kingston with a degree in Mining engineering. In 2008 roy was awarded the engineer’s Medal for entrepreneurship by the Professional engineers of ontario and in 2009 he was given the Metal Mining Society Award by the Canadian Institute of Mining (CIM) In 2012 he was named a Paul Harris fellow by rotary International. In 2017 he was appointed Incoming President elect of the CIM. Also in 2017 nipissing University bestowed upon him an Honorary Doctorate. roy has been active in numerous safety initiatives and in 2013 was appointed to the Province of ontario’s first Prevention Council to advise the government on workplace safety. He currently serves on the CIM Safety Council.

Alistair RossValeDirector, Mining operations

A leader in the mining industry, Alistair ross, Director of Mining (ontario operations), Vale Canada Limited, is known for his passion for mentorship and for helping develop true leadership potential in both individuals and organizations. Before joining Vale in 2017, he provided leadership advice and coaching to Sudbury businesses experiencing rapid growth. ross’s mining career spans six countries across the globe. He was President of Lonmin’s South Africa operations, the world’s third-largest platinum producer. Before joining Lonmin, he spent many years at Inco’s Canadian operations. His early career was spent in the USA in the copper mining world with Phelps Dodge and Magma Copper companies. Prior to coming to north America, he started his career in Africa, working as Production Superintendent at BCL’s Botswana smelter, and as Area Metallurgist for rio Tinto Zimbabwe.

As a Global Mining executive, Michael's focus is the implementation of effective HSeS and risk management programs that direct a clear safety strategy, reduce likelihood of human error, to facilitate the safety of a Company’s workers, the public, and the environment. In 2016, Michael was appointed Global Head of Health, Safety, and environmental Programs for Siberian Coal and energy Company (SUeK). In this role, his responsibility was to advance the Company’s achievement of world-class occupational health and safety standards. His engagement included implementing HSe and risk management programs that support corporate health and safety objectives while ensuring the sustainability of operations. Michael's work entails collaboration with key decision makers to maintain a focus on safety while driving productivity enhancements and delivering financial cost-savings opportunities. Throughout his career, Michael has managed multiple operations, delivering results in operations / production, engineering, government affairs, health and safety, risk management, and regulatory compliance. His career spans broad industry experience in domestic and international mining operations (coal, metal and non-metal, and hard rock), capital construction projects, and rail and port transportation operations. Prior international experience includes leadership for 35,000 global employees across north America (U.S., Canada, Mexico); South America (Colombia, Brazil); Africa (Liberia); eastern europe (Kazakhstan, Ukraine, russia); and europe (United Kingdom, Belgium).

Kim SladePublic Services Health & Safety Association (PSHSA), CanadaDirector emerging Markets and Commercialization

Kim Slade is the Director emerging Markets and Commercialization at Public Services Health and Safety Association. She has a Bachelor of Arts in english and Communications and also has an Adult education Certificate from oISe University of Toronto. Kim is also part of the Canadian Standards Association Technical Committee on occupational Health and Safety Training and currently sits on the technical committee for the development of a Paramedic Psychological Health and Safety in the Workplace. Kim has  been in the field of occupational Health and Safety (oHS) training and education for the past 15 years. This has included securing funding through research grant opportunities and special project funds to develop a variety of products and solutions for the health and safety market including pictograms  and low literacy training for restaurant, retail, industrial and greenhouse sectors, various health and safety training programs and providing leadership as a principal knowledge and co-investigator for the development of a game based application to help leaders improve oHS decision making and skills development.  Kim is currently responsible for PSHSA’s market intelligence, emerging markets and occupational Health and Safety research, including fatigue.

Lora CavuotoUniversity at BuffaloUnited StatesAssistant Professor

Lora Cavuoto is an Assistant Professor in the Industrial and Systems engineering Department at the University at Buffalo. She joined UB in 2012 after receiving her PhD in Industrial and Systems engineering from Virginia Tech. She earned an MS in occupational ergonomics and Safety and a BS in Biomedical engineering from the University of Miami. At UB, Dr. Cavuoto directs the ergonomics and Biomechanics Laboratory. Her current research focuses on quantifying physical exposures and physiological responses in the workplace to identify indicators of fatigue development. Her research work also aims to understand and model the effects of health conditions, particularly obesity and aging, on physical capacity, specifically strength, fatigue, and motor performance. Her work on fatigue and worker safety has recently been funded by the American Society of Safety engineers foundation and the national Institute for occupational Safety and Health.

Ivo GonzalezSherritt International, CanadaDirector, environment Health and Safety

Ivo joined Sherritt International in 2012 and will be the first to admit that he has a lot to learn about the mining industry.

Prior to Sherritt Ivo had 13 years of health safety & environmental (HS&e) experience through progressive leadership positions at a fortune 100 multinational.

During that time he spent 3+ years as an operations manager. In that role he headed a $4.8M manufacturing operations portfolio with more than 200 employees.

He is currently the global leader of HS&e for a small mining company with no mines in Canada but two joint-ventures in developing countries.

He is a champion of continuous improvement systems and has experience leading lean / six sigma transformations. He is a trained lean expert and six sigma green belt.

Ivo’s professional skills and experience includes cross enterprise leadership, strategic decision making, leading change, lean / six sigma, health safety environmental management, and operations management.

Ivo is fluent in english and Spanish but can order a beer in several other languages. He is a proud father of twin girls.

Ward Metzler, Business Director for Central north America and Canada, DuPont Sustainable Solutions brings a wealth of experience and expertise in production, operations, research, and marketing.  He has over 35 years of safety, operations and environmental experience in DuPont.  His present role involves strategic account management as well as leading business development.  Ward Metzler has led major engagements with large oil and gas company to strengthen their risk management processes.  Ward has been able to work with clients to provide them with solutions that integrate with their strategic direction.

Ward is a lead practitioner in the implementation of High Performance Work Systems in DuPont Canada. He led the startup of a complex chemical process in DuPont’s efforts to provide CfC alternatives. His various assignments in Manufacturing have provided Ward with the experience of being accountable for safety and environmental performance.  Many people in DuPont have a paradigm that a recordable Injury or a Lost Workday Case is the worst possible outcome.  The time spent in explosives and in fluorochemicals manufacturing helped him to recognize that a workplace fatality was a possible outcome if the right systems and processes are not in place.

Bill MercerAvalon Advanced Materials, CanadaVice President, exploration

Dr. Mercer has been VP, exploration with Avalon since 2007. Dr. Mercer earned his BSc. in geology from edinburgh University (1968) and a PhD from McMaster University (1975). He enjoyed a 32-year career with the noranda/falconbridge group, holding a series of managerial positions, working on international projects in over 30 countries, ultimately serving as Director-Geology and Geochemistry.

Dr. Mercer served as the President of the Canadian federation of earth Sciences (CfeS), the umbrella organization for all earth science associations in Canada (2008 to 2010), and as President of the Prospectors and Developers Association (2002 to 2004), serving as an executive of the association for eight years. He currently chairs or is a member of several PDAC committees, including the Health and Safety Committee and Geoscience Committee. from 2010-2013 he was a member of the Canadian federal government natural Sciences and engineering research Council grants review committee for earth Sciences, and prior to that a member of the Minister’s national Advisory Board on earth Sciences for the federal natural resources Canada.

In January 2009, Dr. Mercer was the recipient of the David Barr Award from the Association for Mineral exploration in British Columbia for excellence in leadership and innovation in mineral exploration health and safety. He also was honoured with the PDAC Distinguished Service Award in 2006. In 2015, he was the recipient of the “Award for environmental excellence” of the nWT and nunavut Association of Professional engineers and Geoscientists in recognition of his environmental efforts relating to renewable energy use at Avalon’s nechalacho Project in Canada’s northwest Territories.

Dr. Mercer was selected in 2013 to be recipient of the Queen elizabeth II Diamond Jubilee Medal for his service to the Canadian mining industry.

Pat Byrne has been a health and safety professional for over 37 years. He has been the Canadian Director on the board of the International occupational Hygiene Association, an Appeal Commissioner at WorkSafeBC and national President of the Canadian registration Board of occupational Hygiene. Since 2005 Pat has focussed on fatigue management in the mining industry and in professional sports. He pioneered sleep and fatigue management programs in professional sports for teams in all north American and Australian leagues.  In addition his programs have been adopted globally by such diverse organizations as Caterpillar and the US Special operations Command. Pat has worked extensively in the mining industry in Australia and the USA including iron ore, coal, salt, copper and uranium. His early sleep and fatigue monitor the “readiband” was named by the Mining Association of Canada as one of the top “100 innovations in the mining Industry”. Currently, Pat is the research co-ordinator for the Stanford Sleep epidemiology research Center in Palo Alto and provides his expertise to industry and professional sports teams.

Terms and Conditions:1. PAyMenT TerMS: AL Conference Production Ltd. (AL) requires the full payment of the invoiced amount within 7 working days from the issue

date of the invoice. AL reserves the right to refuse entry to any client who has not paid its invoice. The registration fee includes: conference documentation, admission to all conference sessions, lunches and refreshments, admission to networking social events. The registration fee does not include: travel, hotel accommodation, transfers or insurance.

2. HoTeL ACCoMMoDATIon: overnight accommodation is not included in the registration fee. A reduced rate may be available at the hotel hosting the event. The reservation form will be sent to the client after the venue has beenc onfirmed, but no later than one month before the event begins.

3. CAnCeLLATIon By CLIenT: The client has the right to cancel his/her participation in the event. Cancellation must be received by AL in writing, either by mail or fax. If the client cancels with more than one month’s advance notice before the start of the event, AL shall be entitled to retain and charge 50% of the amount payable for participation in the event. If the client cancels with one month’s (or less) advance notice, or fails to attend the event, then the client shall not be entitled to any refund. failure to attend an event shall not excuse a client from owing the full amount of the registration fee. A copy of the conference notes from the event will be sent to the client after the event is over in case of cancellation by the client.

4. CAnCeLLATIon By AL: While every reasonable effort is made to adhere to the advertised program, circumstances can arise which may cause changes in the program, including but not limited to changes in the content, date(s), location or venue, or special features of the planned event. Such circumstances include but are not limited to acts of terrorism, war, extreme weather conditions, compliance with government requests, orders and legal requirements, failure of third party suppliers to timely deliver, and failure to register the minimum target amount of attendees for a given event. AL reserves the right to change the content, date(s), location or venue and /or special features of an event, to merge the event with another event, or to postpone it or cancel it entirely as appropriate under the circumstances. Client agrees that AL shall not be liable for any cost, damage or expense which may be incurred by client as a consequence of the event being so changed, merged, postponed or cancelled and client agrees to hold AL harmless and to indemnify AL in case of liability caused by any such changes, mergers, postponements or cancellations.

5. CAnCeLLATIon of THe eVenT: In case AL cancels an event, then AL may offer the client a full credit up to the amount actually paid by the client to AL. This credit shall be valid for up to one year from the issue date of the invoice to attend any AL-sponsored events. The client shall not be entitled to this credit as a contractual right.

6. CLIenT’S IDenTIfICATIon InforMATIon. By signing of this sales contract and these terms and conditions the client gives full right to AL to share the client’s identification information such as, but not limited to, client’s name, address, email addresses, phone numbers and names of representatives with third parties, which participated on the same event as the client.

7. JUrISDICTIon: This contract shall be governed and construed in accordance with Illinois Law (not including its conflict of laws provisions AL and the client agree that, any dispute in any way arising out of or relating to this contract will be resolved by arbitration using one arbitrator before JAMS or American Arbitration Association in Chicago, Illinois, USA. The parties to this agreement agree that in any arbitration proceeding they may conduct reasonable discovery pursuant to the arbitration rules, that he law of the state Illinois will be the governing law, and any arbitration award will be enforceable in state or federal court.

8. DISPUTeS InVoLVInG CreDIT CArD PAyMenTS: As a condition of AL agreeing to accept your credit card as a approved form of payment, you especially agree to waive any rights you may have under applicable state and federal truth in lending laws or otherwise (including, but not limited to, under your credit’s card issuer’s procedures for resolving such disputed) to receive a temporary credit from your credit card issuer for disputed charges arising from your credit card transaction with AL (commonly referred to as a “charge back”). you agree that any dispute that may arise with respect to any credit card transaction must be addressed directly between you and us and to work in good faith to resolve any such disputed invoices in timely manner. Any dispute that can not be timely resolved to the mutual satisfaction of the parties shall be resolved in accordance with the dispute resolution provisions as contained in this Agreement.

9. CoLLeCTIon/ATTorney’S feeS: The parties agree that in the event that any dispute arises in any way relating to or arising out of this Agreement, the prevailing party in any arbitration or court proceeding will be entitled to recover an award of its attorney’s fees and costs, plus pre and post judgment interest. If we retain the services of a collection agency or attorney to assist in the collection of any amounts due to us under this Agreement, you will pay all expenses incurred by us in such collection efforts.

10. InDeMnIfICATIon: To the fullest extent permitted by the law, you agree to protect, indemnify, defend and hold harmless AL, its owners, managers, partners, subsidiaries, affiliates, officers, directors, employees and agents, from and against any and all claims, losses or damages to persons or property, governmental charges or fines, penalties, and costs (including reasonable attorney’s fees) (collectively “the Claims”), in any way arising out of or relating to the event that is the subject of this contract, and regardless of negligence, included but not limited to, Claims arising out of the negligence, gross negligence or intentional misconduct of AL employees, agents, contractors, and attendees; provided, however, that nothing in this indemnification shall require you to indemnify AL Indemnified parties for that portion of any Claim arising out of the sole negligence, gross negligence or intentional misconduct of the AL parties.

11. oTHer ConDITIonS: Any terms or conditions contained in the client’s acceptance which contradict or are different from the terms and conditions of this registration document shall not become part of the contract unless individually negotiated with AL and expressly accepted by AL.
