Managing Osteoarthritis: Helping The Elderly Maintain Function and Mobility

The effects of osteoarthritis accumulate as people age.

The presence of osteoarthritis significantly predicts whether or not elderly people will become functionally limited in their ability to care for themselves.

Osteoarthritis of the knee and hip joints causes the most difficulty for the elderly because it significantly impacts their ability to conduct their normal activities of daily living, such as walking, cooking, bathing, dressing, using the toilet, and performing household chores.

Joints can become painful, stiff, and swollen.

The resulting pain causes limited motion, reduced physical capability, restriction of social activities, and compromised work capacity.

The interaction of these factors—pain, loss of social contact, and diminished income—can create emotional distress, which further reduces physical activity.

Increased disability results when the affected joints are exercised less, and the elderly begin to lose muscle tone, leading to reduced strength.

Because there is no cure, the therapeutic goal of treatment for osteoarthritis is to minimize the effects of the disease and its consequences over time.

Use of NSAIDs (Pain Killers) Contributes to High Costs

Medications do not cure osteoarthritis but are intended to relieve pain.

AHRQ studies indicate that nonsteroidal anti-inflammatory(NSAIDs) drugs were the medications of choice for osteoarthritis pain until research showed that they affect joint cartilage metabolism, have greater risk of toxicity , can cause upper gastrointestinal bleeding, and may cause or aggravate peptic ulcer disease.

AHRQ study showed that 30-40 percent of all elderly use NSAIDs each year and 10-13 percent of the elderlyuse NSAIDs every day.

AHRQ research further showed that NSAIDs provided only a modest decrease in osteoarthritis pain and little improvement in function, and their association with ulcers, bleeding, and perforation caused a four- to five-fold increase in hospitalizations due to gastrointestinal complications.

Surgical repair and replacement of the knee joints provide durable pain relief and functional improvement in patients with Knee osteoarthritis.

The AHRQ total knee replacement Patient Outcomes Research Team (TKR PORT) showed that, despite the risk of complications, quality of life improves for the elderly after knee replacement surgery

TOTAL HIP JOINT REPLACEMENT SURGERYHip replacement surgery, also called total hip arthroplasty, involves removing a diseased hip joint and replacing it with an artificial joint, called a prosthesis.

Hip prostheses consist of a ball component, made of metal or ceramic, and a socket, which has an insert or liner made of plastic, ceramic or metal.

The implants used in hip replacement are biocompatible — meaning they're designed to be accepted by your body — and they're made to resist corrosion, degradation and wear.

If bone cement is used to fix implant, it is called Cemented THR.In Un cemented THR, implant surface is coated with special material ie Hydroxyappatite (HA) which allows bone ingrowth and fixation.

Hip replacement is typically used for people with hip joint damage from arthritis or an injury.

Followed by rehabilitation, hip replacement can relieve pain and restore range of motion and function of hip joint.

One or both knees may be affected by arthritis. If both knee joints show changes on X-rays and only one knee joint is painful, stiff, and disturbs daily activities of life, then only the painful knee joint should be operated on. If both knees are equally painful, then both should be operated on.

The patient will have only one surgical "event", single anesthesia, and a shorter hospital stay.

Simultaneous Bilateral Total Knee ReplacementIf both knees are equally painful and stiff, it is possible to have both knees replaced during one operation under one anesthesia. The operations are followed by a single rehabilitation period within one hospital stay. Such two simultaneous surgeries are called Simultaneous Bilateral Total Knee Replacement or single sitting bilateral total knee replacement.

The surgery that replaces both damaged knee joints is called bilateral total knee replacements.
