Getting the right care andsupport for you

Care and support is changing and isgetting better.

The changes will happen from April 2015all over England.

They are part of the new Care Act.

These changes will help make sureeveryone gets the care and support theyneed wherever they live.

Care and support means help withthings like:

l washing

l dressing

l eating

l getting out and about

l keeping in touch with friends or family.

These changes could help you if you:

l get care and support at home

l live in a care home.

Care and support whereveryou live

Wherever you live your council must thinkabout the same list of needs when theydecide what help they can give you.

This will make it easier for you to makeplans now and for the future.

And if you move to a different part of thecountry you will still get the care andsupport you need.

More control over decisions

The changes mean you will:

l help to decide the care and supportyou get

l be able to make sure you get the careand support you need

l know how much your care costs

l know how much the council paystowards your care and how the moneyis spent.

Support for all your needs

Everyone’s needs are different. They maybe:

l physical

l mental

l emotional.

A local charity or support network mightbe able to give the support you need.

Your council will put you in touch withorganisations to keep you well and helpyou live independently.
