The goods and services shown in this catalog are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Prior to drafting any contract for goods or services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need arrangement in possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.

It is the policy of this firm that professional fees are paid in full, twenty four hours prior to services being rendered. For your convenience we accept checks, cash, or major credit cards.

BASIC SERVICES AND OVERHEAD OF THE FUNERAL DIRECTOR AND STAFFThis fee is for our basic services and overhead will be added to the total cost of the funeral arrangements you select (this fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains). This fee includes the following:

• Personnel available 24 hours a day, 365 days a year to respond to initial call

• Arrangement conference and follow-up visits

• Coordinating service plans with cemetery crematory and/or other parties involvedin the final disposition of the deceased

• Clerical assistance in the completion of various forms and permits associatedwith a funeral

• Also covers overhead including facility maintenance, equipment and inventorycosts, insurance and administration expenses and governmental compliance $595.00

DIRECT CREMATION (Non-ceremonial Cremation) If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials such as fiberboard or composition materials (with or without an outside covering). The containers we provide include containers made of wood, containers made of fiberboard, and containers composed of other composite materials. See descriptions below. We also offer a selection of cremation oriented caskets. Our charges for direct cremation include basic service of staff transfer of remains to funeral home, securing necessary permits, other preparation and care of the body, sanitary care (refrigeration), and crematory fee.

• Bodies weighing over 300 lbs. will incur a surcharge of $195.00

• Bodies weighing over 400 lbs. will incur a surcharge of $250.00

• Bodies weighing over 451 lbs. call for quote) $700.00 to $1,695.00
