Executive Summary General Ophthalmic Services (GOS): Activity Statistics for England and Wales, Year Ending 31 March 2009 The objective of the General Ophthalmic Services (GOS) is to provide, through community opticians’ practices, preventative and corrective eye care for children, people aged 60 and over, people on low incomes, and those suffering from or pre-disposed to eye disease. This report presents summary information about GOS activity in England and Wales for 2008/09 and on trends since 1999/00. The statistics are derived from data entered onto the Central Ophthalmic Payments System, an NHS Connecting for Health product, accessed via Open Exeter, by Primary Care Trusts (PCTs) in England and Local Health Boards (LHBs) in Wales. The Central Ophthalmic Payments System records General Ophthalmic Service claims, automates the payment process, produces required statistical returns and provides fraud prevention and detection tools.

In the period 1 April 2008 to 31 March 2009: England

• There were 11.278 million NHS sight test claims processed. This is an increase of 231 thousand (2.1 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 563 thousand (5.4 per cent).

• There were 346 thousand domiciliary sight tests, an increase of 17 thousand (5.2 per

cent) on the previous year. Between 2006/07 and 2007/08 the increase was 20 thousand (6.3 per cent).

• 4.138 million NHS optical voucher items were processed, an increase of 187

thousand (4.7 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 186 thousand (5.0 per cent).

• 467 thousand NHS voucher items for repair or replacement were processed. This is

an increase of 4,800 (1.0 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 36 thousand (8.3 per cent).

Wales

• There were 719 thousand NHS sight test claims processed. This is an increase of 18 thousand (2.6 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 25 thousand (3.7 per cent).

• There were 20 thousand domiciliary sight tests, an increase of 1,200 (6.7 per cent) on

the previous year. Between 2006/07 and 2007/08 the decrease was 540 (2.9 per cent).

• 269 thousand NHS optical voucher items were processed, an increase of 13 thousand

(5.0 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 2,800 (1.1 per cent).

• 29 thousand voucher items for repair or replacement were processed, an increase of

634 (2.3 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 145 (0.5 per cent).

Detailed tables at a PCT/LHB level for the information contained in this report and some historical years are provided in Annexes B, C, D and E. The factsheet tool (Annex F) allows the user to view selected summary information for a PCT/LHB, and compare the results against national level results and either its parent Strategic Health Authority/Welsh Region or another PCT/LHB.

Introduction

1999/00.

1. This report has been produced by The NHS Information Centre for health and social care. It shows information about NHS1 ophthalmic activity provided through the General Ophthalmic Services (GOS) in England and Wales in 2008/09 as well as a time series from

2. The NHS Information Centre currently produces two GOS Activity reports and one

GOS Workforce report each year. 3. The summary statistics presented here are derived from payments data provided by

Primary Care Trusts (PCTs) in England and Local Health Boards (LHBs) in Wales, who contract with ophthalmic practitioners to provide NHS sight tests, NHS optical vouchers and vouchers for repairs or replacement.

4. The majority of the data for this report are entered onto the Central Ophthalmic

Payments System, an NHS Connecting for Health product, accessed via Open Exeter, by PCTs and LHBs. The Central Ophthalmic Payments System records General Ophthalmic Service claims, automates the payment process, produces required statistical returns and provides fraud prevention and detection tools.

/vaprodopth/ 6. These data are downloaded and analysed by the NHS Information Centre. Some

PCTs/LHBs do not use the Central Ophthalmic Payments System and submit manual returns.

7. The activity figures presented in this report relate to claim forms that were processed

within the period. The figures may be affected over time by the gradual shift to electronic submission of sight test (GOS1) claim forms. The electronic link was introduced in May 2007 in England and in August 2008 in Wales.

8. For those PCTs/LHBs that use the Central Ophthalmic Payment System, the

submission of forms relating to NHS optical vouchers (GOS3) and vouchers for repairs or replacements (GOS4) is currently entered individually.

9. The report is presented in three main sections: NHS Sight Tests; NHS Optical

Vouchers; and Vouchers for Repair or Replacement. It also contains information on Domiciliary Sight Tests, an estimate of the number of private sight tests conducted, and an estimate of the total number of sight tests (both NHS and private). This report covers NHS ophthalmic activity provided through the General Ophthalmic Services, much of which is undertaken by high-street opticians. It does not cover any private

work delivered by these opticians, although we have used survey data to estimate the number of private sight tests provided towards the end of the report.

10. Total sight tests, NHS optical vouchers, and vouchers for repair or replacement

information included in this report are not sample based as this information is entered onto the Central Ophthalmic Payments System, or returned manually.

11. Information contained in this report on NHS sight tests by patient eligibility is based on

a sample of forms from the majority of organisations, as this information is generally not accurately recorded directly onto the Central Ophthalmic Payments System. PCTs/LHBs are asked to manually provide this information on a two per cent sample of forms by selecting every 50th form and recording the patient eligibility on a separate form that is returned to the NHS Information Centre. This information is then grossed up to produce total figures by PCT/LHB, Strategic Health Authority/Welsh Region, and country.

12. Most organisations provided the patient eligibility information based on a two per cent

sample. Some organisations used different sample sizes and some organisations provided 100 per cent data (direct from the Central Ophthalmic Payments System). A few organisations also returned NHS optical voucher information on patient eligibility based on a sample.

13. All repairs information contained in this report is based on complete information,

entered directly onto the Central Ophthalmic Payments System, or returned manually. 14. Information on NHS Sight Tests and NHS Optical Vouchers by patient eligibility is

approximate, particularly at a local level, as this is based on the eligibility category that is recorded on the claim form. Patients may qualify on more than one criterion, however they would only be recorded against one criterion on the form.

15. In October 2008 two new eligibility categories were introduced for NHS sight tests and

NHS optical vouchers. These were adults receiving income-related Employment and Support Allowance (ESA) and Prisoner on Leave (POL). See annex A for further information.

16. The NHS Information Centre will be undertaking a formal consultation review of its

ophthalmic activity publications over the next few months. This will cover all elements of the publications including content, methodology, and frequency.

17. In advance of the main consultation exercise, we propose to make one change at this

stage. This is to produce the General Ophthalmic Services: Activity Statistics for England and Wales on an annual basis. This publication will cover the information currently collected and published twice a year. If you have any comments on this change, please send them to [email protected] by 19 November 2009.

Sight Tests NHS Sight Tests2

18. The tables in this section present information on NHS sight tests. Patients with a HC3 certificate as part of the low income scheme are entitled to partial funding towards a private sight test. Data relating to HC3s are presented later in this report.

19. Information on patients eligible for NHS sight tests is provided in Annex A. 20. Table 1 below shows the number of NHS sight test claims processed in England and

Wales since 1999/00.

In the year ending 31 March 2009:

• There were 11.278 million NHS sight tests in England. This is an increase of 231 thousand (2.1 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 563 thousand (5.4 per cent).

• There were 719 thousand NHS sight tests in Wales. This is an increase of 18

thousand (2.6 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 25 thousand (3.7 per cent).

22. Table 2 below shows the breakdown of NHS sight tests by patient eligibility and by

country for 2008/09. For the majority of organisations, this information is collected as a two per cent sample, and then grossed up to produce total figures. Further details on sampling are included in Annex A and the individual sample sizes returned by each PCT/LHB are contained within Annex B.

23. Note that patients may qualify for a NHS sight test on more than one criterion.

However, they would only be recorded against one criterion on the form. Where clinical need is one of several qualifying criteria, it is likely to be recorded as the reason for the test as patients with a clinical need generally have a shorter recall period between tests.

24. Table 2 below shows that in 2008/09 in both England and Wales, persons aged 60 and

over accounted for the highest proportion of NHS sight tests at 44.5 per cent in England and 46.4 per cent in Wales. Children, aged 15 and under, were the second highest for both England and Wales at 20.5 per cent and 21.0 per cent respectively.

27. Tables 4a and 4b show the number of NHS sight tests per 100,000 population for England and Wales. These figures are based on the population size of each individual category. Table 4a shows that in 2008/09, there were 22 thousand NHS sight tests for every 100,000 people in England, an increase of 451 from 2007/08. Table 4b shows that in Wales this figure was 24 thousand, an increase of 606 from 2007/08.

28. Annex C contains this information for 2007/08 and 2008/09 at SHA/Welsh Region and

PCT/LHB level. Information for children 0-15 and adults 60+ for earlier years is not available at this local level due to concerns over the quality of some local level data by patient eligibility for those earlier years. Information on all NHS sight tests per 100,000 population is available at this local level from 2003/04 onwards.

The recall period for NHS sight tests varies according to patient eligibility. Not all groups are entitled to an NHS sight test every 12 months. Further details are contained in Annex A.

Patients may qualify for an NHS sight test on more than one criterion. However, they would only be recorded against one criterion on the form. Patients are more likely to be recorded according to their clinical need rather than their age. For example, a patient aged over 60, with glaucoma is likely to be recorded in the glaucoma category only. The counts for these age groups are therefore underestimates.

In England in 2008/09, 0.9 per cent of NHS sight tests were conducted by Ophthalmic Medical Practitioners, a decrease of 0.7 percentage points on the previous year. Domiciliary Sight Tests 31. The vast majority of NHS sight tests are conducted at ophthalmic practitioners’

premises. A small proportion of tests (3.1 per cent in England and 2.7 per cent in Wales) were conducted away from ophthalmic premises in 2008/09. These include sight tests carried out at people’s homes and at residential homes. Domiciliary sight tests performed on either a single patient at an address, or where the patient is the first or second to be seen, are reimbursed at a higher rate. Third or subsequent patients visited at the same address are reimbursed at a lower rate.

32. Tables 6a, 6b, 6c, and 6d show the number and percentage of domiciliary sight tests by

domiciliary visit rate, by country, 1999/00 to 2008/09. There were 346 thousand domiciliary sight tests in England in 2008/09, an increase of 17 thousand (5.2 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 20 thousand (6.3 per cent).

Private Sight Tests and Private Domiciliary Sight Tests with Partial Help

35. Patients with HC3 certificates as part of the low-income scheme are entitled to varying degrees of financial help with private sight tests. Table 8a below shows that in England in 2008/09, PCTs made partial payments towards 3,300 private sight tests, of which 202 included a private domiciliary sight tests. Table 8b below shows that in the same period for Wales, LHBs made partial payments towards 227 private sight tests, of which 20 included a private domiciliary sight tests.

NHS Optical Vouchers 36. The voucher scheme was introduced in July 1986. Under the scheme, patients who

had previously been eligible to have NHS spectacles were given NHS optical vouchers to put towards buying their own choice of spectacles, or having new lenses fitted to their existing frames. From April 1988, NHS optical vouchers could also be put towards the cost of contact lenses. Details of which patients are eligible for NHS optical vouchers are provided in Annex A. Patients may receive a voucher for more than one pair of spectacles.

37. Due to different eligibility regulations for NHS sight tests and NHS optical vouchers, the

eligibility category recorded for NHS optical vouchers may differ from that recorded as the categorisation for the NHS sight test.

38. Table 9 below shows the number of NHS optical voucher items processed in England

and Wales. It shows that in 2008/09:

• 4.138 million NHS optical voucher items were processed in England, an increase of 187 thousand (4.7 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 186 thousand (5.0 per cent).

• In Wales 269 thousand NHS optical voucher items were processed, an increase of 13

thousand (5.0 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 2,800 (1.1 per cent).

42. Tables 12a and 12b show the number of NHS optical voucher items processed per 100,000 population for England and Wales. These figures are based on the population size of each individual category. In 2008/09 in England, 8,100 NHS optical vouchers were processed for every 100,000 people, an increase of 367 from the previous year. This figure for Wales in 2008/09 was 9,000, an increase of 432 from 2007/08.

43. Annex D contains a national and SHA/Welsh Region level time series of all NHS optical

vouchers processed per 100,000 population from 1996/97 and at PCT/LHB level from 2003/04. It also contains information for children 0-15 per 100,000 population for 2007/08 and 2008/09 at a SHA/Welsh Region and PCT/LHB level.

Vouchers for Repair or Replacement 46. Patients under the age of 16 can obtain a voucher towards the cost of repair or

replacement for damaged or lost spectacles or contact lenses. Patients over 16 must satisfy certain criteria (that the loss or damage was due to illness; that help is not available through warranty, insurance or after sales service; and that the patient would be entitled to a voucher for new glasses or contact lenses) before a voucher towards the cost of repair or replacement is issued.

47. Table 15 below shows the number of NHS voucher items for repair or replacement

processed in England and Wales since 1999/00. The number of NHS voucher items for repair or replacement were processed in England in 2008/09 was 467 thousand. This is an increase of 4,800 (1.0 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 36 thousand (8.3 per cent).

48. In Wales, 29 thousand voucher items for repair or replacement were processed, an

increase of 634 (2.3 per cent) on the previous year. Between 2006/07 and 2007/08 the increase was 145 (0.5 per cent).

Private Sight Tests and Total Sight Tests 53. It is possible to make an approximate estimate of the number of private sight tests by

applying the ratio of private to NHS sight tests obtained from the Sight Tests Volume and Workforce Survey3 to the number of NHS sight tests reported earlier.

54. The Sight Tests Volume and Workforce Survey for Great Britain (GB) have been

carried out by a Technical Sub Committee (TSC) from 1993. The latest survey relates to 2005/06. The committee comprises of the Department of Health and professional representatives from the Federation of Ophthalmic and Dispensing Opticians, Association of Optometrists, and the British Medical Association with the Chair and Secretariat provided by The NHS Information Centre for health and social care.

55. The NHS Information Centre conducted the latest Sight Tests Volume and Workforce

Survey for 2005/06 over a 12 week period from August to November 2005. The total number of sight tests performed in Great Britain during 2005/06 was estimated using the known number of sight tests actually paid for by primary care organisations (PCOs) over the 12 month period, and the estimated ratio of private to NHS sight tests from the survey.

56. The survey asked each practitioner to record, over a one week period, the ages of all

patients and if their test was paid for by the NHS or was paid for privately. 57. According to this survey, 68.6 per cent of all sight tests in Great Britain4 were NHS

funded in 2005/06, with the remainder being paid for privately. Applying this ratio to the number of NHS sight tests reported in England and Wales produces a total estimated number of sight tests in 2008/09 by country.

58. For England, it is estimated that there were 16.5 million sight tests in total, with 5.2

million of these being privately funded. 59. For Wales, it is estimated that there were one million sight tests in total, with 300

thousand of these being privately funded. 60. The proportion of sight tests funded by the NHS in Great Britain increased from 54.2

per cent in 1996/97 to 68.6 per cent in 2005/06. Part of this increase may be due to patients aged 60 or over becoming eligible for NHS sight tests from April 1999.

Annex A: Notes

NHS Sight Tests, NHS Optical Vouchers and Repairs or Replacements 1. In April 1989, NHS sight tests were restricted to the following eligible groups:

• Children aged under 16; • Students in full time education aged 16 - 18; • Adults receiving Income Support and their partners; • Adults receiving Family Credit (FC) and their partners (no longer applies); • Patients on low income holding health benefits certificate HC2 and their partners; • Glaucoma and diabetes sufferers; • Patients registered blind or partially sighted; • Patients requiring complex lenses; • Close relatives aged 40+ of a glaucoma sufferer;

2. Subsequent additions to the eligible groups were:

• Adults receiving a Disability Working Allowance (DWA) (from April 1995) (no longer applies);

• Adults receiving income based Job Seekers Allowance (from October 1996); • Patients aged 60 or over (from April 1999); • Adults receiving full Working Families Tax Credit or Disabled Persons Tax Credit, or

credit reduced by £70 or less (from October 1999), replaced FC and DWA; • Have been advised by an ophthalmologist that you are at risk of glaucoma (from

February 2001); • Adults receiving Pension Credits guarantee credit (from October 2003); • Adults receiving Income-related Employment and Support Allowance (ESA) (from

October 2008); • Prisoner on Leave (POL) (from October 2008).

3. From 27 October 2008 Income-related Employment and Support Allowance was introduced for some people who would previously have received Income Support. Those receiving ESA who would previously have received Income Support will be entitled to NHS sight tests and NHS optical vouchers. This category has been added to new GOS forms but these forms are not yet in full circulation. Following guidance from the Department of Health, where a person is entitled to an NHS sight test under ESA but the practitioner is using a form without this category, they are to input the individual as receiving an NHS sight test on grounds of income support. As such, they have been included within income support in our analysis.

4. Prisoners on leave from prison will also be a criteria on GOS forms entitled to NHS

sight tests and NHS optical vouchers from 27 October 2008. This category has been shown separately in our analysis.

8. Most organisations provided this information based on a two per cent sample. Some

organisations used different sample sizes and some organisations provided 100 per cent data (direct from the Central Ophthalmic Payments System). A few organisations also returned NHS optical voucher information based on a sample.

9. In July 1986 the NHS voucher scheme was restricted to the following:

Optometrists and Ophthalmic Medical Practitioners 12. An Optometrist, also known as Ophthalmic Optician (OO) is qualified to test eyesight

and to prescribe and dispense spectacles. Their training enables them to recognise abnormalities and diseases of the eye. They are registered with the General Ophthalmic Council (GOC).

13. An Ophthalmic Medical Practitioner (OMP) is a qualified doctor who specialises in eyes

and eye care. In addition to their medical skills, they are qualified to test eyesight and prescribe spectacles. They are registered with the General Medical Council (GMC).
