Annual Enrollment is your once-a-year opportunity to enroll in or change your Vision Care Benefits.* Be sure to understand your options so you are enrolled in the plan that is best for you. If you are happy with your current coverage, no action is necessary. You will retain the same coverage you currently have. You only need to take action if you wish to change plans. What are my options? You can select from two options: GE Vision Basic Option or GE Vision Plus Option. New in 2012 - Vision care improvements Effective January 1, 2012, under the Vision Basic Option, the co-payment will be eliminated for in-network purchases of up to four boxes (up from two) of collection contact lenses a year for those under 19, and four boxes every other year for those over 19. The $20 co-pay for scratch resistant coating on eyeglass lenses will also be eliminated. Also, both the Vision Basic Option and the Vision Plus Option will be expanded to include new coverage for plan participants diagnosed with “low vision” who require certain types of vision aids. GE Vision Basic Option The Vision Basic Option pays benefits, according to a schedule, for routine eye exams, new lenses, frames or new contact lenses. Eligible expenses are covered every other year for participants age 19 and over. (Annually for participants under age 19.) GE Vision Plus Option Under the Vision Plus Option, routine eye exams, new lenses, frames or new contact lenses are covered every year. Additionally, this option provides higher reimbursement for certain allowances and a lifetime maximum allowance of $250 per eye for laser vision surgery. How much does each cost? If you are an eligible full-time employee, the Company pays the entire cost of the Vision Basic Option coverage for you and your eligible dependents, so there is no cost to you. The Vision Plus Option can be elected and contributions apply. The weekly contribution rates for full-time employees for Vision Plus are: 1-Person coverage $1.96; 2-Person coverage $3.91; and 3-or-More-Persons coverage $5.87. When and how do I enroll? During Annual Enrollment, you can enroll online at benefits.ge.com 24 hours a day, 7 days a week. The online tools at benefits.ge.com give you access to all your current benefits data and offer comparison information. You can also enroll by calling the GE Benefits Center at 1-800-252-5259, Monday to Friday, 7 a.m. to 7 p.m., Eastern time. The GE Benefits Center will also be open 9 a.m. to 1 p.m. Saturday October 29, November 5 and 12; and from 7 a.m. to 7 p.m. on Veterans Day, November 11.
