BOWLING EXCURSION – (REINDEER GAMES) I give permission for my child(ren) to attend an excursion to a public venue, Shoalhaven City Lanes Bomaderry. Date: Friday 20th December 2019. Location: LOT 5 Narang Rd, Bomaderry NSW 2541 Ph. (02) 4422 1200 Method of Transport: Children will travel to the venue by bus (Shoalbus). Duration of Excursion: 10:00am – 1:00pm approx. PARENT BOOKING TERMS & PERMISSION NOTE I give permission for my child(ren) to participate in the above selected activities under the supervision of Educators at COOSH Cambewarra.

I confirm that my child(ren) hold a current enrolment with COOSH Cambewarra.

I understand that half payment of fees must be made at time of placing this booking and that the remaining balance is to be paid by the first day of my child(ren)s attendance.

I understand my child(ren) must wear enclosed shoes daily & please NO singlets (for safety reasons).

I understand I am to ensure my child(ren) brings a hat, lunch and a drink bottle daily.

I understand that inappropriate and/or violent behaviour exhibited by my child(ren) towards other children and/or Educators will NOT be tolerated. I understand that should an incident occur Educators will notify an ‘Authorised Contact’ to collect the child from COOSH/excursion venue immediately.

I understand incursion and excursion times indicated are approximate and may change.

I understand that transport will be by private bus unless stated otherwise.

I understand that the maximum number of children attending an excursion is 30 and number of staff is 2 unless stated otherwise.

I understand that the staff:child ratios for excursions will be based on 1:15.

I am aware that risk assessments have been prepared for each excursion and incursion and these are available from COOSH at the commencement of each day or as requested.

I understand that I MUST check my myGov account to make sure that my childcare account has not ceased (not checking may result in loss of Child Care Subsidy).

I understand that COOSH will only be operational between 8am & 5pm during the school holidays (7am – 6pm School Staff Development Days from 2020) & Educators are unable to accept children at COOSH outside of these hours. Should children be present after the 5.00pm closing time, a Late Pickup Fee of $2.50 per minute (not covered by CCS) will be applied.
