• HEALTH ANd WELLNESS PROGRAMS We provide free health and wellness programs on healthy eating, physical activity, emotional wellness and risk factor management. In addition to programs offered at the Hants Community Hospital, we also work closely with community partners to offer our programs in a variety of locations.

• WELLNESS NAVIGATION Navigatorsworkwithyouone-on-onetofindtherightservicestohelp.We’rehealthprofessionals who know health care, the community and government systems. We work with individuals and families.

• CHRONIC dISEASE MANAGEMENT We provide individual and group programs for chronic disease management for people with diabetes, high blood pressure, high cholesterol, and/or those who need nutrition education.

• COMPLEX CARE We also provide individual appointments to support complex care for citizens who have three or morespecificchronicconditions.Wewillworkwithyouandyourregularhealthcareproviderto manage your health care.

Anyone can self-refer to our health and wellness programs. We ask that if you are referring yourself to our complex care programs that you have the support of your family doctor or nurse practitioner.

The Hants Health and Wellness Team is a health care team that promotes emotional and physical wellness and self management of chronic conditions. We do this by helping individuals to gain knowledge, skills, and confidencetotakechargeoftheirownhealth.Weprovidethefollowingcoreservicesforcitizensandtheirfamilies in the Hants area:
