CLINICAL RESPONSIBILITIES

Apart from general urology my clinical responsibilities include the following

urological subspecialties:

• Laparoscopic urological surgery: I have a regular practice in laparoscopic

surgery for a variety of indications, including nephrectomy for benign and

malignant disease, pyeloplasty, partial nephrectomy, renal cryoablation and

adrenalectomy.

• Stone disease:

• Extracorporeal shock-wave lithotripsy: The Bristol Urological Institute

has a Storz lithotriptor for the treatment of renal and ureteral calculi, the

only fixed-site machine in the Southwest of England. The lithotriptor

provides the backbone to a comprehensive stone treatment and

prevention service.

• Joint clinic in metabolic stone disease with Dr. Helen Campbell of

nephrology.

• Percutaneous and ureteroscopic stone extraction: Bristol serves as a

referral centre for complex kidney stone disease. I carry out minimally

invasive treatment of urinary calculi in all locations using the latest

nephroscopes and ureteroscopes, both rigid and flexible, as well as the

holmium:YAG laser.

• Minimally invasive treatment of upper urinary tract transitional cell carcinoma:

Selected patients with upper tract tumors can be treated endoscopically, thereby

preserving their kidneys. Patients are referred from across the UK to the Bristol

Urological Institute for diagnosis and treatment.

• Minimally invasive treatment of upper urinary tract obstruction: Carefully

selected patients with ureteropelvic junction (UPJ) obstruction and ureteral

strictures can be treated endoscopically. Laparoscopic pyeloplasty has emerged

as our preferred treatment option for patients with primary and secondary UPJ

obstruction. We have one of the most extensive experiences with this procedure

in the UK.

OTHER PROFESSIONAL ACTIVITIES

2011 – 2015: HONORARY SECRETARY, BRITISH ASSOCIATION OF UROLOGICAL

SURGEONS. I served as an officer and Trustee of BAUS with primary responsibility

for overseeing the Annual Scientific Meeting. I was regularly involved in issues

involving urological services and the NHS, such as commissioning of services and

training. I have taken a leadership role for the following areas: the public reporting

of individual surgeons’ outcomes; meeting with the Department of Health regarding

the setting of tariffs, especially in endourology and kidney cancer; the BAUS Section

of Endourology; liaising with NICE; and UROLINK, which supports urology in the

developing world.

2014 – PRESENT: CHAIRMAN, BAUS DATA AND AUDIT COMMITTEE. I oversee the

annual publication of surgeon’s outcomes, which now includes nephrectomy,

percutaneous nephrolithotomy, radical prostatectomy, radical cystectomy,

urethroplasty, and surgery for stress urinary incontinence.

2008 - 2011: LEAD CONSULTANT, DEPARTMENT OF UROLOGY, BRISTOL UROLOGICAL

INSTITUTE, NORTH BRISTOL NHS TRUST. I helped to facilitate a merger of the two

departments of urology in Bristol. I carried out a detailed review and redesign of

waiting list management. This process had tangible results, including a reduction in

waiting times for review cystoscopy from 6 months overdue to within the target

month; setting internal targets for urgent non-cancer cases; and instituting a safe

transfer of care policy.

Our department has a da Vinci robot, principally to do robotic radical prostatectomy

(RRP). Concerns were raised about the safe introduction of RRP by low-volume

surgeons, so I developed guidelines for training and oversaw the safe expansion of

the technique.

2005 – 2008: COUNCIL MEMBER, BRITISH ASSOCIATION OF UROLOGICAL SURGEONS.

While serving on BAUS Council, I collaborated to publish the BAUS Guidelines for

laparoscopic training as well as for mentorship (see publications below). I have also

contributed to the development of patient information for recovery from

laparoscopic nephrectomy, in collaboration with the Royal College of Surgeons of

England and the Department of Health. I have contributed towards the BAUS

annual meeting for each of the past 14 years.

2004 - 13: BJU International (Journal). I served as a trustee and Honorary Treasurer

(2010-13) of the Executive Committee, which oversees the publication of the journal.

As Honorary Treasurer of the BJU International Company (2010-13), a charity

dedicated to promoting urological education and research, I was responsible for an

annual budget of roughly £2m and reported to the Trustees. This role involved

regular communication with the Chief Executive and Chairman. I had to educate

myself in financial matters as the Honorary Treasurer of the BJUI during challenging

times for publishers. I reported to the Trustees 4 times per year and led the

appointment of new auditors and an investment advisor.

2003 – PRESENT: NATIONAL INSTITUTE FOR CLINICAL EXCELLENCE (NICE). I have been

asked to provide expert advice on a wide range of interventional procedures. From

2008 to 2012, I served on the Interventional Procedures Advisory Committee.

2000- PRESENT: LAPAROSCOPIC TRAINING. I have participated in laparoscopic

training through BAUS-sponsored courses as well as acting as a mentor for

consultant urologists. I have run a laparoscopy lecture course at the BAUS annual

meeting and performed live surgery in over 15 courses at national and international

meetings.

As the regional training center for urology, NBT typically attracts specialist

registrars at the end of their training. I have trained 26 specialist registrars and 11

fellows in endourology; 14 of them have been appointed as consultants with an

interest in endourology.

I have acted as a mentor for 30 consultant urologists, assisting in 67 cases at hospitals

around the UK and abroad. The mentoring process involves demonstrating

laparoscopic procedures in Bristol, then tutoring consultants as they carry out the

same procedures at their own hospital.

I have organized courses in laparoscopic renal cryoablation and acted as a preceptor

for 10 consultants.

2005- 2007: BAUS LAPAROSCOPY TRAINER. I oversaw the hands-on teaching courses

for BAUS in the UK and in Europe. Roughly 9 courses took place each year.

2005-PRESENT: FACULTY OF THE EUROPEAN SOCIETY FOR URO-TECHNOLOGY (ESUT),

an endourology subspecialty group within the European Association of Urology

which focuses on laparoscopic training.

2009-PRESENT: CLINICAL RESEARCH OFFICE OF THE ENDOUROLOGICAL SOCIETY

(CROES): Committee member for ureteroscopic procedures. I help CROES design

and run international studies and registries which have led to over 30 publications.

Department of Surgery.

2000- 2007: CLINICAL GOVERNANCE, DEPARTMENT OF UROLOGY. This commitment

involved preparing and overseeing our clinical governance sessions including

Morbidity and Mortality meetings. Within this role, I oversaw clinical audit,

reporting of adverse events, and appraisal. I have attended courses on clinical audit

and appraisal.

2000- 2011: BAUS SECTION OF ENDOUROLOGY. I served on the audit committee,

which has carried out an audit on laparoscopic nephrectomy and the equipment

available for the treatment of stones. I have developed a database for the treatment

of PUJ obstruction as well as for upper tract TCC, which are used for the prospective

analysis of outcomes. I have overseen the development and reporting of the BAUS

national audit of laparoscopic nephrectomy (see publications below). As mentioned

above, I have overseen laparoscopic training courses for the Section.

1997- PRESENT: ENDOUROLOGICAL SOCIETY. I served on the Board of Directors (2010-

14) which oversees an annual meeting, teaching courses, and the Journal of

Endourology. I have served on the organizing committee/faculty on 15 occasions for

the World Congress of Endourology. I performed live surgery during the 2005 and

2009 meetings. I was on the organizing committee for the 2015 meeting to be held in

London.

2004- PRESENT: COCHRANE REVIEWS. Reviewer for Cochrane reviews. I have co-

My research activity includes the following aspects of upper urinary tract disorders:

• The qualitative aspects of surgeon-reported outcomes.

• Cryoablation and partial nephrectomy in the treatment of kidney tumors.

• The diagnosis and minimally invasive management of upper urinary tract

transitional cell carcinoma.

• The evaluation and treatment of ureteropelvic junction obstruction.

• The development of new techniques in laparoscopic urological surgery.

• The minimally invasive management of stone disease.

• The investigation of mechanisms responsible for stone fragmentation from

extracorporeal shock wave lithotripsy (SWL).

• The role of anatomic factors in stone clearance following SWL.

• Comparison of irrigating fluids during TURP.

• Endoluminal ultrasound in the evaluation of UPJ obstruction and the staging

of upper tract transitional cell carcinoma.

• The quality of life of patients with urinary stone disease, especially in

patients requiring ureteral stents.

• The evaluation of ureteral physiology and pharmacology.

• The evaluation of medical expulsive therapy for ureteral stones.
