Create a referral in KidTraks (Go to “Home Based Services” and then “Family Preservation”). Choose a provider.
We work with families in every Indiana County.
Provider makes first contact with family.
You can expect us to make contact within 24 hours.*
Receive Protective Factors Survey from the provider.
Receive monthly report from provider.
We also provide an updated Treatment Plan.
Review case for re-referral.
We evaluate the family’s situation to advise if a re-referral is recommended.
Receive the acceptance or rejection from selected provider.
You will receive a response from us within 24 hours.
Receive the initial assessment and safety plan from provider.
Receive weekly updates, including in-home visit report, from provider.
Our weekly reports include additional safety checks.
Receive follow-up Protective Factors Surveys from provider.
