Petitioner, thru counsel, and unto this Honorable Court, respectfully avers the following:

1. Petitioner is of age, single, Filipino, and a resident of Caridad Sur, Matalom, Leyte

2.Petioner has been a bonafide resident of Matalom, Leyte since birth at 1990;3. Petitioners present name is Ututino Bantoto B. Aho;

4. That such a name is ridiculous and has caused embarrassment to petitioner through the years;

5. Petitioner request that his present name be changed to GOODO B. AHO.

WHEREFORE, it is respectfully prayed that, after due notice and publication in accordance with the Rules of Court, and hearing, this Honorable Court adjudge that the petitioners name of UTUTINO BANTOTO B. AHO be changed to GOODO B. AHO.
