Form I-9 Instructions For Completion Please read through carefully. Any inaccuracies will be returned for revision. U.S. Citizenship and Immigration Services requires all employees to complete the Form I-9, Employment Eligibility Verification, to establish identity and employment eligibility. In cases when an employee cannot present original documents to a SNHU employee (due to distance), we request the assistance of an Authorized Representative for the completion of the employer’s section (Section 2) of the Form I-9 on our behalf. The employee is responsible for bringing the Form I-9, along with their forms of identification, to an Authorized Representative. After completion, the employee must mail the form to Southern New Hampshire University. Copies of the identification documents do not need to be sent. Prior to mailing, both page one and page two must be completed as shown below. Please note: The employee is responsible for completing page one of the document, and the Authorized Representative is responsible for page two. About the Authorized Representative: There are many professionals that can act as an authorized representative. SNHU does not specify a specific individual. The selected individual is made at the discretion of the employee (to exclude family and friends). Some options include but are not limited to: HR representatives, colleagues, administrative personnel, librarians or staff at your state unemployment or workforce agency. A notary is not required. Due to distance, our prospective employee has asked you to complete the form I-9 on Southern New Hampshire University's behalf. As the Authorized Representative, you are stating that you have examined the identification document(s) presented and that the document(s) appear to be genuine (on their face) while in the individual's presence. Section 2 must be completed in its entirety, with the exception of employment date, to be adequate as verification of employment eligibility. Please note that if you are a Notary, your Notary stamp or seal should not be placed on this form.

The individual can present either:Any one document from List A Two documents, one from List B (identity) AND one from List C (eligibility)

*For further clarification on acceptable documents, refer to page 3 of the I-9 form. View the unexpired, original document(s)the individual presents. The only exception is a certified copy of a birth certificate.

Record the employee’s name and citizenship/immigration status from Page 1.

Record document title(s), issuing authority, document number(s), and the expiration date from original document(s) supplied by the individual. All documents have this information, with the exception that List C documents may not have n expirations date.

NOTE: You may use abbreviations for commonly used documents, such as DL for Driver's License and SSC for Social Security Card.

*Please omit the first day of employment.Attest to examining the documents provided by filling out the certification section only. You must sign, date, provide your title as "Authorized Representative", and print your last and first name. SNHU's name and address should remain as the organization's name and address.

Section 3, "Re-verification and Rehires", should remain blank for HR use. The Authorized Representative should not sign on the bottom line.
