(i) Copies of documents relating to identifying the same as different, independent and identifiable premises of the existing industrial undertaking.
(ii) whether metering system of adjacent additional unit is duly tested and sealed at site by the licensee?
(iii) month wise consumption of last 36 months may be given of the existing premises.
3. Whether the capital investment in the form of building, plant and machinery is more than 100 cores in additional unit? If not, give details of investment value of all existing plants in the State as on date of commencement of manufacture or production of the additional unit.

(VII) Whether energy is supplied to any other person. If so, give the following details:-
(a) Rate at which power is supplied. (b) Number of separate meters
installed.
(c) Purpose for which the energy is consumed by such consumer.
(d) A copy of the agreement entered with such consumer for sale of power.

1. Date of installation of each generating set.
2. Date of starting generation of electrical energy from each generating set.
