O Our facility in Forest, VA was designed and built specifically for those with Alzheimer's or dementia. This residence creates a home-like environment that helps to minimize the confusion and agitation that often accompanies these illnesses. This is a secured residence with locking delayed egress exit doors for the security of each resident. Each of the hallways are colored coded to give visual cues to residents allowing them to move about freely and independently. This security allows families to enjoy peace of mind knowing that their loved oneone is living in an environment that fosters their independence, nurtures their self-esteem and overall maximizes their quality of life. The quality of life issues for each resident is defined by unique needs and personal interests.

All of these factors are reviewed at the time of admission, and on an annual basis, to develop an individualized continuing care plan that maximizes their quality of life while addressing both mental and physical needs. Residents with memory impairments may not always be able to express themselves in words. Therefore, it is important for the staff that take care of them to detect likes or dislikes by knowing their body language, through slight expressions or just a touch. At Runk & Pratt of Forest we do this for our residents by treating them as a unique individual, with their own personal interests and abilities taken into consideration when designing a personalized plan of care to maximize and promote dignity and choice.

Care Fees

Admissions Fee: $1000.00 fee for all admissions to include furniture, linens, bedspread, laundry, housekeeping, and establishment of medical chart. An annual fee of $500.00 will be invoiced annually, thereafter. EXCEPTION: residents admitting the fourth quarter of the year are not invoiced the following year.

Assisted Living base monthly fee: Private: $3295.00 Semi-Private: $2895.00

Assisted Living additional levels of care monthly fees:

Level I - $205 per month

1. Requires assistance with bathing 2. Requires supervision of dressing 3. Short-term memory loss, but does not wander 4. Feeds self 5. Remembers to use bathroom but requires assistance with cleanup and has occasional incontinence

Level II - $410 per month

1. Requires dressing, bathing and grooming assistance 2. Incontinent – requires staff to change diaper and cleanup after incontinence 3. Feed self but supervision may be required 4. May require one person to assist or standby supervision on ambulation

Level III - $615 per month

1. Dependent for bathing 2. Dependent for dressing 3. Dependent for eating 4. Requires 2 people to assist for ambulation 5. Requires someone to push wheelchair to and from meals 6. Requires assistance with transfer 7. Incontinent

Guest Services: Guest Meals: $5.00 Breakfast - $5.00 Lunch – $5.00 Dinner (Please place your order by 10:00 am same day)

Respite Care: Private room: $155.00 per day – includes furnished room, all meals, med management, linens, laundry, housekeeping
