Felodipine Improves Work and Exercise Capacity In patients with chronic stable effort angina The haemodynamic effects of felodipine were assessed in 15 outpatients with chronic stable effort angina pectoris. Patients were randomly assigned to receive felodipine 5 or 10mg, or placebo on 3 different days with a 24-hour interval between each treatment. Both doses of felodipine significantly increased heart rate and decreased sitting and supine systolic and diastolic BP at rest compared with placebo 3 hours after drug administration. At 10 hours, the differences were not statistically significant. Three hours after drug administration, systolic BP in sitting and supine positions was significantly lower with felodipine 10mg than with felodoipine 5mg. During exercise, 3 hours after drug administration, heart rate at ischaemic threshold significantly increased from 107 beats/min at baseline to 123 beats/min after felodipine 5mg daily and to 125 beats/min after felodipine 10mg daily. Heart rate at peak exercise increased from 122 beats/min to 137 beats/min with each dose (p < 0.001). Compared with placebo, both doses of felodipine significantly increased pressure-rate product 3 hours after drug administration. At 10 hours, the differences in heart rate and BP between the 3 treatment groups were not statistically significant. Three hours after drug intake, exercise duration significantly increased from 375 seconds at baseline to 479 and 470 seconds after felodipine 5 and 10mg respectively. At 10 hours, exercise duration was significantly higher after felodipine 10mg than after felodipine 5mg daily (418 vs 367 seconds). Total work to onset of angina and to peak exercise, 10 hours after drug administration, were significantly greater after the larger dose of felodipine (2305 vs 1911 kpm and 2716 vs 2288 kpm, respectively). Three patients receiving felodipine 5mg daily, 1 patient receiving felodipine 10mg daily and 1 receiving placebo experienced headache. One patient receiving felodipine 10mg daily experienced asthenia and skin rash. The authors concluded that felodipine 5 and 10mg improved exercise capacity up to 10 hours in patients with chronic stable effort angina pectoris and that the drug was well tolerated. Scardl S.
