Pinkalicious! Age: 4 - 5 years (4 years by June 1, 2020) Fee: $110Dates: June 15 - 19, 9:30 - 11:30 amPinkalicious and the Pink Drink! Pinkalicious Crazy Hair Day! Join Pinkalicious on her adventures! Make fun art projects, dress up, learn about healthy eating, have a tea party, and end camp with a parade around the Museum! Fun for all!

Pete the Cat Age: 4 - 5 years (4 years by June 1, 2020)Fee: $110Dates: June 8 - 12, 9:30 - 11:30 am Meet Pete, the groovy blue cat! No matter where he goes, Pete the Cat always keeps his cool! He loves surfing, playing baseball and guitar, spending time with his friends, and trying new things. We will explore some of Pete’s favorite stories and add a new one: Pete the Cat’s World Tour. Not to worry with Pete...It’s All Good!

Mix and Make Age: 4 - 5 years (4 years by June 1, 2020)Fee: $90 Dates: Section A: June 23 - 26, 9:30 - 11:30 am (Camp meets Tuesday - Friday)Dates: Section B: June 23 - 26, 1:30 - 3:30 pm (Camp meets Tuesday - Friday)Mix, mash, roll, pat, mold, sculpt, and more! This tactile adventure is for young scientists and artists who like to get their hands a little messy. Mix your own air-dry play dough for building, and discover what sticky slimes can be made from ingredients in your kitchen.

Angelina Ballerina Age: 4 - 5 years (4 years by June 1, 2020) Fee: $110Dates: July 20 - 24, 9:30 - 11:30 amCalling all mouselings who have dreams of becoming a real ballerina! Join us in the ballet studio for a week of beginning ballet class. We will also take a look at real tutus, pointe shoes, and hear some stories of some of Angelina’s favorite ballets. Dancers should wear activewear or tights, a leotard, ballet shoes, or bare feet.

Turtley Radical Dance Camp Age: 4 - 5 years (4 years by June 1, 2020) Fee: $110Dates: July 27 - 31, 9:30 - 11:30 amMake a splash during this ocean-themed week of science AND Dance! Swim with the fish, build a coral reef, and dress up like a mermaid. Each day will include movement activities as well as science and art. The Nutcracker SweetAge: 6 – 8 years (6 years by June 1, 2020)Fee: $110Dates: July 6 - 10, 9:30 - 11:30 amJoin Clara and the Sugar Plum Fairy for a magical ballet journey through the land of sweets. We’ll dance to the music of the Nutcracker, try on costumes, create props, and finish the day by making a sweet treat to eat! We will also have a mini performance on the last day of camp. Make your own shirt, and end the week with a tie-dye party! Campers must wear clothes that can get messy!

Enchanted Engineering Age: 7 – 10 years (age 7 by June 1, 2020)Fee: $90Dates: June 9 - 12, 9:30 - 11:30 am (Camp runs Tues - Fri) Use your STEM skills to fix fractured fairy tales. Which simple machines can rescue Rapunzel from her tower? Can you create a wolf-proof house for the Three Little Pigs? Campers will try to solve problems from a new fairy tale every day.

All About Slime Age: 7 – 10 years (age 7 by June 1, 2020)Fee: $90Dates: Section A: July 14 - 17, 9:30 - 11:30 am (Camp runs Tues - Fri) Dates: Section B: July 14 - 17, 1:30 - 3:30 pm (Camp runs Tues - Fri) Let’s get messy! Learn how to make all kinds of slime with all kinds of ingredients! Use science and chemistry to figure out what makes slime so sticky! Dress to make a mess please!

Wizard Camp Age: 7 – 10 years (age 7 by June 1, 2020)Fee: $110Dates: July 27 - 31, 1:30 - 3:30 pmDo you love Harry Potter? Hop on your broom and fly to our wizarding school. We’ll sort into houses and then the fun begins. We’ll make wands, play wizarding games, drink butterbeer, and have a magical time! Wizarding attire is encouraged. Mischief Managed!

American Girl Doll Camp! Age: 7-10 years (Must be 7 by June 1, 2020)Fee: $140Dates: July 27 - 31, 9:30 am - NoonBring your American Girl doll and get ready for an exciting week of cra�ing, cooking, and exploring history with and for your doll. Each day will feature a specific doll’s story and focus on her particular time period. The week will also conclude with a tea party for you and your doll!

This week will be filled with learning choreography based on Broadway’s most exciting shows! If this waiver is not signed, your child will not be allowed to participate. If you are signing for a minor, both parents must sign.

For class cancellations, families may choose to either have class credit applied to their account or receive a refund minus 10% processing fee.

Cancellation by parents for classes must take place by noon on the Friday prior to the first day of class.

a. You will need to register for a City of Bettendorf account before you can sign up for Family Museum summer camps. Visit familymuseum.org/registration to sign up for an account.

b. To view camps visit familymuseum.org/registration. Registration for camps will begin at 12:00 am on the morning of April 18. Full payment is due at time of registration.

Mail-in registration must be postmarked no earlier than Saturday, April 18, 2020.

Permission to Participate INDEMNIFICATION WAIVER OF PARENTAL CLAIMS

THE CITY OF BETTENDORF REQUIRES THIS FORM BE SIGNED BEFORE PARTICIPATION IN A CITY SPONSORED ACTIVITY. IF THIS FORM IS NOT SIGNED, YOU OR YOUR CHILD WILL NOT BE ALLOWED TO PARTICIPATE. DO NOT SIGN IF YOU HAVE ANY QUESTIONS.

If you are signing on behalf of a minor, both parents must sign. In the case of a single parent home, the custodial parent must sign. If you are registering your child for class electronically, completion of the registration will be considered your consent and agreement with this document.

1. You agree to release, waive, and hold harmless the City of Bettendorf, the Bettendorf Community School District, agents, employees, volunteers, and contractors of either entity (Collectively referred to as “RELEASEES”) from any and all liability which may accrue to you from your child’s participation or your participation in the following activity:

2. This release waives all of your claims whatsoever, known or unknown, which may arise by virtue of your child’s participation or your participation in the activity, including injury or death, and damage to property, however such claim may arise, including but not limited to breaches of duty (such as breach of the duty of care) and acts of current or future negligence by Releasees, their respective o�cers, employees, or agents, including, but not limited to, negligent care, supervision, or control. This release waives any of your claims whatsoever against the Releasees arising from the actions of any other participant in the activity or any other third party. Further, this release covers all activities immediately before and after participation, including transporta-tion to or from the event, and waiting for rides from City or School facilities after the event.

3. You agree to defend and hold harmless Releasees, their respective o�cers, employees, or agents against any claim, cause, loss, cost, or damage whatsoever, including attorney fees, that arise from the above-described activity or your child’s participation in the above-described activity, including, but not limited to damages for injury to your child or another person. This means, in part, that in the event something happens to your child during his/her participation in the activity, and your child decides to sue the Releasees, that you agree to pay any sums awarded to your child, and to pay the Releasees’ attorney fees incurred in the defense of any litigation. You are also agreeing to pay for any damage caused by your child during participation in the activity.

4. Participation involves a risk of injury. By signing this form, you represent that you have considered the risks of your child’s participation or your participation in the activity, have obtained any medical clearance necessary to participate, and you or your child are able to participate without harm to your child, yourself or others. You represent you or your child will use any program equipment with care for himself/herself, yourself, other participants, and the surroundings. You have evaluated the risks of participation in the event by your child, and grant permission for your child to participate.

5. In the event of injury, program supervisors shall administer basic �rst aid, and shall summon emergency services via 911. Supervisors are not trained to provide detailed medical care, and shall not, without prior agreement, provide medications. All costs of emergency care are the responsibility of the participant/ participant’s legal guardian(s). NO INSURANCE IS PROVIDED BY THE CITY FOR INJURY TO PARTICIPANTS. All claims for reimbursement of medical care costs, including emergency transportation, for you or your child are hereby waived.

6. Parents or others may take audio or video recordings of the participants. The City does not supervise or restrict recordings of public activities by third parties. You agree that photos or videos may be used in promotional materials produced by the City to encourage participation in the activity.

7. You and your child agree to follow all rules of conduct established in conjunction with the activity. Failure to follow the rules will result in removal from the activity, and you and your child agree that any program fees paid will be kept and not refunded. In the event of a problem while engaged in the activity, notify the activity manager/supervisor.

8. This Permission to Participate and Hold Harmless is given in partial consideration of you and your child being allowed to participate in the activity described, and binds yourself, your personal representatives, and any heirs or assigns.

9. In the event a court of competent jurisdiction declares any part of this indemni�cation agreement to be invalid, the balance of the agreement shall be deemed to survive and be binding upon you.
