Who are the staff?The Certified Diabetes Educator (CDE) is a Registered Dietitian (RD) and Licensed Dietitian Nutritionist (LDN). She speaks English and some Spanish.
How do I receive CCI services?A CCI provider’s referral is required to participate in nutrition services. If you don’t have a referral, you must ask for one from your CCI provider and bring it to your appointment.
Call 1-866-877-7258 to make an appointment. The clinic staff may call you the day before your appointment to confirm you are coming. If for any reason you must cancel, please call 1-866-877-7258 to let us know and to reschedule.
What will happen at my first nutrition services visit?Remember to come 15 minutes early. At your first visit, you will be enrolled as a CCI health care nutrition services patient. The dietitian will call your name and you will be shown to the office. Your follow-up visits for CCI health care nutrition services will be at the medical discretion of the dietitian.

An individualized meal plan and a FREE gift – a glucose meter with meter supplies – will be issued to each eligible patient upon completion of all classes. To continue receiving FREE glucose meter supplies and ongoing
