Enrollment for your voluntary benefits begins at 9:00 a.m. Eastern Time (ET) November 8, 2010 and ends at 6:00 p.m. ET November 19, 2010. You will be able to enroll, disenroll or make changes to your EyeMed Vision Care, Hyatt Legal Plans and/or OneBeacon Identity Management Services for 2011 during these dates only. NOTE: The open enrollment period for your 2011 Alcatel-Lucent health and welfare benefits coverage is also scheduled to begin at 9:00 a.m. ET, on November 8, 2010 and end at 6:00 p.m. on November 19, 2010. That open enrollment period is separate from the Voluntary Benefits Enrollment. To enroll in your health and welfare benefits coverage, please visit the Your Benefits Resources Web site at http://resources.hewitt.com/alcatel-lucent. Payments for these benefits are made through convenient post-tax payroll deductions. Payment is broken out evenly over your 2011 payroll cycle. Welcome to Your 2011 Voluntary Benefits Open EnrollmentIt’s time to enroll in your voluntary benefits for 2011.

Please note on November 19, 2010, 6:00 p.m. ET, the open enrollment period will end, after which you will not be able to enroll in coverage for vision, identity management or legal services, change your coverage level or cancel coverage for 2011, unless you experience a qualified status change.

What’s Changing

Voluntary vision care coverage will be extended to adult children up to the end of the month in which they become age 26, without regard to student or marital status. Such adult children must also not be eligible to enroll in another employer’s coverage. (This change does not apply to the children of domestic partners, whose coverage eligibility age limit will continue to be the end of the month in which the adult child turns 24 if he or she is a full time student, otherwise end of the month in which the adult child turns 20 if they are not a full time student.)

You may enroll newly eligible adult children in voluntary vision care coverage during the annual open enrollment period if they meet the new eligibility requirements. Coverage will be effective January 1, 2011.

All other eligibility requirements still apply. If you are unsure whether a dependent is eligible for coverage, you can view the full eligibility requirements at www.addedbenefitsaccess.com.

Note: The change to dependent child eligibility does NOT apply to Legal or Identity Management Services.

There are two options for you to choose from:

Both programs provide you with vision care and

prescription eyewear through a vast network that

includes independent providers, as well as retail

locations such as Sears Optical, Pearle Vision,

Target Optical and JC Penney Optical.

ID cards will will be mailed to new participants in

January. If you are continuing your coverage from last

year, you will not receive a new ID card.

Value added features> Members will receive a 20% discount on items not

covered by the plan at in network providers, which

may not be combined with any other discounts or

promotional offers. The discount does not apply to

EyeMed Provider’s professional services, or contact

lenses. Retail prices may vary by location.

EyeMed Vision Care

IMPOrTaNT: Personalized ID cards are printed with the employee’s name only; eligible covered dependents should use the card for identification purposes. Member ID will be the employee’s HRID.

> At an in network provider, once the funded benefit

has been used, members also receive a 40% discount

off a 2nd complete eyeglass pair purchase and a 15%

discount off conventional contact lenses.

> Members also receive 15% off the retail price or 5%

off the promotional price for Lasik or PRK from the US

Laser Network, owned and operated by LCA Vision.

Why vision care coverage?

Regular eye exams are important to the overall health

of your family. A comprehensive eye exam can be the

first indicator of health concerns such as diabetes, high

blood pressure and glaucoma. Your vision plan offers

coverage for eye exams, lenses, frames and contact

lenses. This valuable coverage is available to you and

your eligible dependents.

How do I locate a provider?

Visit www.addedbenefitsaccess.com and select

Benefit Providers under Vision Care to locate a provider

in your area.

How do I know what is covered under my vision care

coverage vs. my medical plan?

Your EyeMed benefit provides coverage for routine eye

care only. If you have a condition that would require

treatment with medication or possible surgery, these may

be handled by your physician through your medical plan.

Here is a list of the most common conditions of the eye

that may be covered under your medical plan:

> Glaucoma > Retinal Detachment

> Conjunctivitis > Retinitis Pigmentosa

> Diabetic Retinopathy > Strabismus

> Macular Degeneration > Uveitis

What if I decide to see a non-participating provider?

If you choose to use a non-participating provider, you

will be required to pay in-full at the time of service and

obtain itemized receipts. A copy of your receipts and a

completed claim form should be submitted to EyeMed

for reimbursement. You will be reimbursed up to the

amounts listed on your schedule of benefits. Claim forms

can be found at www.addedbenefitsaccess.com.

What does a routine eye exam entail?

EyeMed Vision Care’s panel of optometrists and

ophthalmologists conduct comprehensive eye

examinations to diagnose or detect existing conditions

of the eye and vision system. The tests and procedures

performed may vary by patient because of age or

specific conditions, but typically include an assessment,

diagnosis and treatment plan.

All attorney fees for covered services are paid

to choose from. You’ll pay no deductibles or

Single Bureau Credit Monitoring provides a

sophisticated, comprehensive monitoring system to

alert you to credit score inquiries, new credit requests,

and other activity affecting your credit file. The service

will provide immediate e-mail notification of any

changes to your credit records. In an event of

suspicious activity you can take advantage of the

Identity Theft Resolution services outlined above.

aIM asset recovery is a unique resource that is part

of Identity Management Services. It is designed to help

you in recovering unclaimed funds. Funds can come

from a variety of sources including: foreclosures, estates,

insurance proceeds, uncashed checks or stale-dated

checks, tax refunds and overpayments, dormant bank

accounts, and lost stock certificates.
