A. Chief Engineer and Deck Officers to be reviewed by Master whilst Engineer Officers to be
reviewed by Chief Engineer.
B. ln the case of Engineer Officers, the Master must countersign the report and add anyadditional comments.
C. The Master and Chief Engineer are to be reviewed by the Ship Manager.
D. A report must be completed whenever an officer or rating leaves the ship, or every 6 months.
E. The report should be forwarded to the Ship Manager every six months or at the end of contractperiod.
F. The Master must discuss any shortcomings with any crew raised outside this forum during thecourse of their contract.
G. Any training requirements the Master deems necessary should be intimated in the general
comments/training requirements section of this form.
