To recommend priority outcomes to launch the first outcome networks by considering the following criteria:
– Needs exist– Data exists to practice continuous improvement– Momentum exists– Staff and facilitation exists– Finally, the combination of areas is not concentrated in one
section of the continuum

Criteria Score (0-10)
Need exists: using the best available data, there is a significant need to improve the outcome
Data exists to practice continuous improvement: and data can be collected for a significant portion of the target audience and cuts across sectors
Staff and facilitation support exists: Backbone or lead convening organization has capacity to staff and facilitate networks.
Momentum exists: resources or appropriate partners engaged, ready to work, and poised to achieve early wins.
Scoring
Each criterion must be scored A maximum of 10 points may be allotted per
criterion:– 0 to 3 points: does not meet or minimally addresses
criterion requirements– 3 to 7 points: partially meets criterion with some
deficiencies– 8 to 10 points: Substantially of fully meets criterion
requirementsPlease complete the supporting comments section so that we
can understand the rationale for your scores.
