Introduction The appointment of an inaugural Victor Smorgon Chair of Rehabilitation was made through a collaboration of Epworth HealthCare and Monash University in July 2009. The Epworth Monash Rehabilitation Medicine Unit (EMReM) which has been designated as a research arm of Epworth Rehabilitation, commenced in July 2010. In line with the objectives of the chair of rehabilitation medicine, individuals have been supported in higher level degrees and research projects. This is the fourth annual report of the EMReM unit and outlines major research projects, papers published in refereed journals, grants awarded and conference presentations. Throughout the past year, the EMReM team (consisting of 21 individuals) have been productive in our research efforts. Staff have been involved in 21 various research projects, produced 21 papers published in refereed journals and collectively gained $147,673 in grants. In addition, staff have presented research at 52 conferences and have had 17 poster presentations.

Snapshot of EMReM Progress Number of Research Projects Since June 2013, EMReM has supported 21 projects of various types (four of which have been completed within the last 12 months). EMReM has particularly started to be involved in clinical trial and registrar projects. These include:-

Melanie Drummond: The Consequences of Olfactory Impairment following Traumatic Brain Injury

Objectives The aims of this study are to: To identify the incidence of OI (as measured by the PST and the UPSIT) following TBI in a consecutive sample of adults admitted to the Brain Injury Rehabilitation Program at Epworth Hospital, Richmond Campus in Victoria, Australia To investigate the natural progression of olfactory dysfunction following TBI in the context of recovery (6 + 12 months post injury) To identify acute factors that are associated with long term olfactory outcome Completion of the study will also facilitate the development of a ‘best practice’ protocol to facilitate reliable screening for the presence of olfactory impairment in individuals who have sustained a traumatic brain injury (TBI). Babak Farr: Investigating the outcomes of multi-trauma rehabilitation in a stand-alone facility compared to an acute hospital based rehabilitation

facility Objectives This study will be a retrospective audit of routinely collected outcome data at Epworth HealthCare. The aim of this project is to

investigate whether moving a multi-trauma rehabilitation unit from an acute hospital to a freestanding centre has impacted the patient outcomes of the rehabilitation program. The outcomes of interest include: - length of stay, changes in functional independence measures (FIM change), type of injuries and rates of transfer back to acute care. A secondary aim is to compare outcomes (namely length of stay and FIM change) at each site with benchmark and national averages. Bridget Hill: Development of a Patient-Report Outcome Measure for the assessment of adults with traumatic brachial plexus Injury Objectives The aim of this project is to develop a new patient-report outcome measure to assess functional outcome in adults following

Nathan Johns: Rehabilitation following total knee replacement to improve cardiovascular fitness, function and quality of life in the elderly – a pilot study

Objectives The pilot study aims to demonstrate that a 6-week aerobic exercise based rehabilitation program can improve cardiovascular fitness, function and quality of life. This pilot study will also establish that this new program is practical to run and administer in preparation for a larger Randomised Controlled Trial of the same program which will assess the impact of improved fitness on longer term mortality.

Margaret Mealings: Participation at school following TBI

Objectives The aim of this research project is to explore the perspectives of students returning to study following brain injury, in particular to explore the impact on their sense of self and well-being in the context of their school participation. The project will be a longitudinal, mixed methods study engaging with participants over a period of 2 years.

Elizabeth Moore: The influence of adjunctive therapy on outcome following botulinum neurotoxin (BoNTA) injection for focal spasticity in adults with neurological conditions

Objectives This project aims to investigate the influence of adjunctive therapy on outcome following BoNTA injection for focal spasticity in adults with neurological conditions. The group of adults will be provided with a prescribed program from an experienced clinician following their injection designed to maximise their outcome. The rate of adherence to the prescribed program will be examined to determine if adherence to therapy improves outcome. This pilot study will contribute to the limited body of evidence for adjunctive therapy following BoNTA injection and more importantly influence the direction of future research studies. More specifically the 3 aims of the study are:

1) To compare adherence rates to a prescribed therapy program between those who achieved their predetermined rehabilitation goals and those who failed to do so 3 months following BoNTA injection.

2) To determine if there is a difference between non-functional and functional goal attainment 3 months following BoNTA injection.

3) To describe the content and dosage of adjunctive therapies provided and the relationship to the identified goal contribute to the limited body of evidence for adjunctive therapy following BoNTA injection

Peter New: A Study of patient flow through inpatient rehabilitation and potential for improvements

Objectives The aims of this thesis are to study access and discharge barriers for patients admitted into rehabilitation. The primary objectives are: Measure the time sequence for key process for patients referred and accepted into the rehabilitation units to explore if there

are inefficiencies that can be improved upon. Measure the prevalence of ‘exit block’ for patients from inpatient rehabilitation. Where barriers for discharge are identified,

to record the cause of the barrier and the time it took for the barrier to be resolved, and to develop prediction models for length of stay in hospital and the occurrence of barriers to discharge.

Calculate the impact on access for inpatient beds in rehabilitation and length of stay from a number of simulation models testing hypothetical scenarios to address the major exit blocks identified by way of alternate care pathways.

Knee Arthroplasty Objectives Outcomes assessment in the field of arthroplasty rehabilitation will continue to increase in importance as global trends in

osteoarthritis and arthroplasty procedures continue to rise. A unified framework of functioning that will serve as common language is necessary. Knowing what relevant outcomes to assess paves the way for better and improved tools of assessment. It is envisaged that the ICF will have a positive influence on outcomes assessment in this field. The primary aim of this study is to use the ICF Osteoarthritis core set as starting point in developing a clinical measure of functioning in persons undergoing hip and knee joint replacement. Traumatic Brain Injury Objectives The purpose of this study is to compare functional outcomes of patients aged 18 to 40 years old to patients aged 65 years old and

over, who have been admitted to Epworth Rehabilitation, Richmond, Victoria following a severe traumatic brain injury (TBI). Outcome measures which will be used include the functional independence measure (FIM) score, length of stay and discharge destination. This study aims to identify if there is a significant difference in recovery during the inpatient phase after severe TBI in people 65 years old and over. Pearl Chung: Use of the International Classification of Functioning, Disability and Health (ICF) to describe caregiver issues in Acquired Brain Injury

Objectives The aims of the study are to measure the effect size and variance of CSI and CRA among caregivers of patients with brain tumour (pwBT) and patients with stroke (pwS) in the Australian population, for testing the hypothesis that inpatient rehabilitation reduces caregiver strain for caregivers of pwBT and pwS.

Brinda Thirugnanam: Investigating the relevance of information provided to the Person Responsible during family meetings for patients with a Traumatic Brain Injury

Objectives The aim of this project is to evaluate the relevance of information provided during family meetings by the treating rehabilitation team to the person responsible for patients with a traumatic brain injury. The second part of this project aims to assess the understanding of language used by the treating rehabilitation team during one family meeting. This will be assessed by asking two independent individuals from a non medical background to evaluate a transcript from one of the family meetings and identify complex words used. Jeremy Buckmaster and Casey Brownlie: A pilot study to evaluate a Road Trauma Education Program

Objectives To determine the effectiveness of a road trauma program in changing the attitudes towards road safety of secondary school students. As clinicians working within the Acquired Brain Injury Rehabilitation unit at Epworth HealthCare, we recognise the significant impact and far reaching consequences that road trauma has on not only patients but their families and their extended community. Since 2009, it is recognised that patients admitted post road trauma were typically aged under twenty-five with the highest sub group being males aged nineteen to twenty one. This supports consideration of educational programs targeting adolescents. The current study aims to evaluate the effectiveness of an existing road trauma program aimed at school aged students. It also aims to determine the effectiveness of the existing road trauma program with the addition of personal accounts of trauma from rehabilitation staff and TBI patients. Objectives This will be a pilot study investigating the nature of the circadian sleep wake cycle and sleep architecture for patients in PTA compared to post-PTA resolution following a moderate to severe TBI. The aims of this (pilot) study are to gain essential information from a small cohort to allow the design of a larger scale study.

Objectives The study objective is to evaluate the long-term safety of naldemedine for the treatment of constipation due to opioid therapy. John Olver: Retrospective Audit of Medication Use in Patients who have suffered Traumatic Brain Injury and have been admitted to the ERR Traumatic

Brain Unit Rehabilitation Unit

Objectives The aim of the present study is to rationalize the use of medication and review practice in line with evidence-based guidelines. Francisco et al. (2007) reported that for neurobehavioral impairments post traumatic brain injury (TBI) randomised control trials do not exist for every scenario and therefore consensus and best practice guidelines are needed. Therefore, the use of medication and practice will be measured against Francisco et al. report of current medication preferences for managing neurobehavioral sequelae of TBI. An additional aim is to utilize the database as a teaching tool regarding medication use post traumatic brain injury. This may result in a reduction in the use of potential harmful psychoactive drugs in patients with traumatic brain injury.

John Olver: The use of a Post Stroke Checklist (PSC) to identify persistent long-term problems amongst post stroke survivors Objectives This research project aims to identify the frequency of commonly reported long-term problems in Australian stroke survivors using a

scale known as the Post Stroke Checklist (PSC). The PSC consists of 11 long term issues that patients can experience post stroke (e.g. pain, mood, cognitive, communication etc) and identifies the appropriate course of action for each issue. The PSC has been designed to identify common long-term issues and enable a more appropriate mechanism for care for post stroke survivors. A secondary aim is to determine patients’ overall satisfaction with the PSC. This study intends to recruit 300 patients who were admitted to the Acute Stroke Unit at the Alfred Hospital or the Acute Stroke Program at Epworth HealthCare. John Olver: To Establish a Database Recording Patient Outcomes Post Botulinum Toxin Injection for Spasticity from Chronic Neurological Conditions Objectives The present study has collected data since September 2009 when the Spasticity Clinic commenced. The Spasticity Clinic runs bi-

weekly where patients are reviewed and evaluated. Treatment is then initiated which may involve physiotherapy, occupational therapy, splinting or the use of Botulinum Toxin if spasticity is seen to be a significant problem. Patients are then reviewed at 8 weeks after the treatment starts to determine their response and measure their outcomes. Baseline and follow-up data are collected through the Spasticity Management Clinic form which is as per normal patient evaluation. The study aims to optimize treatment protocols including muscle selection and dose ranges to achieve better outcomes and greater goal achievement for patients. Michael Ponsford: Audit of Elective Joint Replacement and Hip Fracture Rehabilitation Outcomes

Objectives To monitor complications and measure functional outcome of patients coming to rehabilitation after hip and knee replacement and hip fracture. We will also be looking at establishing a protocol for prophylaxis of thromboembolism. In addition, it is expected the current study will assist in optimizing future rehabilitation programs to achieve best functional outcomes e.g. length of stay, discharge destination and complications.

John Olver: A Phase III multicentre double-blind prospective randomized placebo controlled study assessing the efficacy and safety of Dysport used for the treatment of lower limb spasticity in adult subjects with hemiparesis due to stroke or traumatic brain injury

Objectives The primary study objective is to assess the efficacy of Dysport compared to placebo at Week 4 on the change from baseline in the gastrocnemius-soleus complex (GSC) muscle tone (knee extended) in hemiparetic subjects with lower limb spasticity due to stroke or traumatic brain injury. The secondary study objectives will include assessments of the efficacy of Dysport compared to placebo on changes in the Physician’s Global Assessment (PGA). Fran Wise: Attitudes to Obesity in Cardiac Rehabilitation Health Professionals

Objectives Studies have suggested that health professionals’ negative attitudes can generate disparities in the delivery of health services and interfere with the implementation of effective obesity prevention efforts. The aim of this project is to identify the attitudes and beliefs of Cardiac Rehabilitation health professionals toward overweight/obese patients, and compare these to a cohort of non-Cardiac Rehabilitation health professionals working in other areas of rehabilitation. All clinical rehabilitation staff at Epworth and Caulfield Hospitals, in addition to members of the Victorian Association of Cardiac Rehabilitation, will be invited to complete an anonymous on-line survey that includes questionnaires measuring beliefs and attitudes regarding obesity, in addition to a measure of empathy.

John Olver Professor Olver developed a 6-week inaugural fellowship in traumatic brain injury (TBI) rehabilitation at Epworth HealthCare (which commenced in February 2014). This is a new program introduced at Epworth HealthCare where attendees are introduced to the various facets of multidisciplinary rehabilitation. There have been two attendees to date, both of which reside internationally in Malaysia (2014-onwards)
