Routine high-speed imaging Detection of even small fractures or bleeding in an acute care scenario is a challenge in CT imaging. With the unique Siemens SureView™ concept, in combination with a 38-mm detector width, trade-offs between scan speed and image quality are not necessary – long scan ranges at sub-millimeter collimation are no longer difficult. Iterative reconstruction With SAFIRE (Sinogram Affirmed Iterative Reconstruction), Siemens introduces the first raw-data-based iterative reconstruction. SAFIRE achieves up to 60%* dose reduc- tion for a wide range of applications or delivers superior image quality. However, it also introduced certain limitations resulting from the increased complexity in image acquisition, reconstruction procedures, and algorithms. But there are innovative ways to overcome such pitfalls. The solution Siemens offers is the fast spiral image reconstruction software SureView. With SureView,

Siemens CT users do not need to choose between scanning at high speed or with high quality. With the SureView recon-struction algorithm you can eliminate this trade-off and achieve both speed and quality in every scan. High-quality imaging at low doseTo facilitate consistent image quality at any pitch even for long scans, Siemens invented SureView – the dedicated spiral image reconstruction algorithm. SureView features easy-to-use scan protocols, in which the

SureView user only needs to select the scan range, mAs, scan time, and slice width. All other parameters are automatically calculated by the scanner. The result is consistent image quality and simple, easy-to-learn dose selection techniques. In addition, high power reserves are available and allow for up to 100 seconds.

CARE Dose4D, an unparalleled combination of maximum image quality at minimum dose, is an example of the Siemens philosophy of providing a wide range of dose-reduction solutions in computed tomography. Because every patient is unique in terms of size, weight, and anatomy, we developed a fully-automated dose management system that takes all of these factors into consideration. Users are

CARE Dose4D able to select more curves to adapt the dose modulation more precisely, and there is a special dose-modulation curve for children. The tube current is adapted in real time and according to the anatomy of each individual patient’s organs. CARE Dose4D can evaluate for attenuation and calculate the appropriate axial tube- current profiles in lateral and AP directions. Axial tube-current variation and angular online tube-current modulation take place in real time during the scan.

iTRIM (Iterative Temporal Resolution Improvement Method) is designed to improve temporal resolution of cardiac CT images. This iterative image reconstruction algorithm improves the temporal resolution by 20%, effectively reducing motion artifacts in CT images while maintaining a very good overall image quality and low image noise. iTRIM is based on the observation that the presence of motion

iTRIM* artifacts does not significantly change the histogram of a CT image. This information is used to reconstruct an image from less than half a turn of data. First, a conventional cardiac partial scan weighted filtered back projection (WFBP) reconstruction is performed, resulting in a temporal resolution equivalent to 180º of CT data. Then, an iterative reconstruction algorithm using only a subset of the full 180° cardiac data set is started. The iterative loop has two steps: First, the image is updated with the projection data subset using the SART

(Simultaneous Algebraic Reconstruction Technique) iterative reconstruction framework. After each SART iteration, the HU value of each pixel is adjusted according to the respective histogram of the lower temporal resolution image. Then, a motion detection technique is used after the iteration. The iTRIM image from the final iteration of the reconstruction is combined with the conventional 180° WFBP image. In regions that exhibit motion, the iTRIM image is used for the final image; whereas in static regions, the WFBP image is used.

Siemens has a long history of exceptional expertise in detector technology. From scintillators and photodiodes to electronics, Siemens state-of-the-art innovations make the difference in detectors where only a few thousandths of a millimeter are significant. The Ultra Fast Ceramic (UFC) detector system is the central element of all Siemens scanners and part of the technology that make innovations such as the SOMATOM

UFC Detector Definition Flash possible. Many clinical innovations have been achieved due to this component alone. Speed, image quality, and low dose are what Siemens UFC detectors make possible. Conventional ceramic scintillator material has a long afterglow effect. UFC scintillators excel in fast decay behavior and have an extremely short afterglow time. As CT systems become faster, the role of image quality becomes more important. UFC scintillators acquire large patient volumes quickly and provide high image resolution.

UFC scintillators allow for the lowest possible dose. Due to their luminous efficiency and optimized wavelength, the detectors enable the applied X-ray dose to be optionally evaluated according to the patient‘s anatomy.

SAFIRE (Sinogram Affirmed Iterative Reconstruction) debuts the utilization of the full dose-saving potential of iterative reconstruction in clinical practice. Now, raw data (visualized in the so-called sinogram) is actually being used in the image improvement process. After initial reconstruction using weighted filtered back projection (WFBP), a first iterative reconstruction loop is performed. The

SAFIRE CT images are retransferred to raw data, which models all relevant geometrical properties of the CT scanner. This step reproduces CT raw data like a real scanner. By comparing this synthetic raw-data with the acquired data, deviations are identified. A further iteration loop compares the images with homologous reference data. This procedure can be regarded as validating (or affirming) the current images. An up- dated image is then again reconstructed, using the detected deviation information. In each iteration, a dynamic raw-data-based

noise model is applied that allows for reduction of image noise without noticeable loss of sharpness. This optimization process makes even better use of the diagnostic information contained in the raw data. Using multiple iterations, geometrical imperfections of the WFBP are corrected in addition to incrementally reducing image noise. Thus, SAFIRE enables a radiation dose reduction of up to 60% or improved image quality in regards to contrast, sharpness, and noise.

CT Cardio-Vascular Engine CT Cardio-Vascular Engine combines dedicated scanner functionality with powerful diagnostic tools, providing a smooth workflow for the evaluation of the most common cardiovascular diseases. With the new iTRIM algorithm, a high temporal resolution as low as 195 msec is achieved to freeze cardiac motion. The disease-specific workflow design covers a wide variety of cardiovascular examinations, from quantification of coronary artery disease to pre-procedural planning.

CT Acute Care EngineThe CT Acute Care Engine combines dedicated functionality with tailored, disease-oriented workflows for lifesaving decisions when every second counts. The Engine covers a wide variety of acute situations, from abdominal imaging, trauma assessment and fast triple rule-out, to neurovascular and stroke imaging. The CT Acute Care Engine Pro now also features the newly released syngo.CT Dynamic Angio, which facilitates bone-free dynamic vessel evaluation.

The information about SAFIRE (Sinogram Affirmed Iterative Reconstruction) is being provided for planning purposes. The product is pending 510(k) review, and is not yet commercially available in the U.S. In clinical practice, the use of SAFIRE may reduce CT patient dose depending on the clinical task, patient size, anatomical location, and clinical practice. A consultation with a radiologist and a physicist should be made to deter-mine the appropriate dose to obtain diagnostic image quality for the particular clinical task.
