1) Please Complete the entire Registration Packet. Do not leave anything blank.

2) Circle your 1st, 2l,d, 3rd, and 4th choice (if applicable). Please follow this for all programs forwhich you want to register.

3) Please return the entire Registration Packet TO THE FRONT OFFICE STAFF ONLY withthe annual S50 registration fee. The front office staff will review the application to makesure everything is completed.

4) Registration Packets will not be accepted without the annual $50 registration.
