What is a Long Term Condition?

‘Long term Conditions are those conditions that cannot, at present, be cured, but can be controlled by medication and other therapies’ (David Colin-Thorne, National Clinical Director for Primary Care)

High quality services for people with long term conditions should support people to develop confidence and competence in managing the challenges of living with their condition(s) in order to have a better quality of life, better clinical outcomes and make more appropriate use of resources.
