2013-2014 RegistrationGeneral Instructions for New & Returning Students

PLEASE READ this entire booklet before completing registration materials.

RETURNING ELMS STUDENTS

Currently enrolled students have Preferred Registration Status until February 28, 2013. After that date, Our Lady of the Elms School will hold Open Registration for new students. In order to be Preferred Registered for School Year 2013-2014, we require all forms and fees to be received by Our Lady of the Elms Business Office by February 28, 2013. Please note, the registration fee for returning students increases by $150 after February 28, 2013.

SIBLINGS OF CURRENT ELMS STUDENTSNew students who are siblings of a current Elms student must make application through the Admissions Office and meet Admissions criteria. Once accepted, siblings receive priority consideration. If the sibling has not yet been accepted, do not include this student in your attached forms and fees. Please contact High School/Middle School Admissions at 330.867.0918 or PreK-Gr. 6 Admissions at 330.752.2507 immediately, if you are considering the registration of one or more new siblings from your family in order to initiate application.

INCOMING FRESHMAN

According to the Association of Catholic Admissions Directors (ACAD) of the Diocese of Cleveland, incoming ninth graders may not register at a Catholic high school until Feb 28, 2013. The Elms will hold Freshman Registration on Saturday, March 2, 2013.

NEW ELMS STUDENTS

Upon acceptance, new students (excluding incoming freshman) should imme-diately complete the enclosed registration forms and return them along with applicable fees to the Business Office.

All forms must be mailed to the Business Office. An envelope is provided for your convenience. Faxed forms are not acceptable since original signatures are required. All forms are to be completed in full, signed where appropriate, and returned to the Business Office in the enclosed return enve-lope. Completed forms must be accompanied by a check made payable to Our Lady of the Elms or credit card information.* Failure to do so will delay your new student’s registration. *The Elms accepts Visa, MasterCard, Discover and American Express. A 3% handling fee will be added.

REGISTRATION FORMS

STUDENT REGISTRATION FORM 2013-14

TUITION MANAGEMENT SYSTEMS (TMS) ENROLLMENT FORM

Parents of new and returning students must complete, sign, and return the reg-istration form. The form must be completed in full, including all information on the back.

TMS is a monthly payment plan plan available to all families. Options include 12 in-stallments beginning in June or 10 installments beginning in August. You will receive a statement in the mail monthly from TMS or you may choose to have automatic bank withdrawal. Do not complete section four unless you prefer the automatic bank withdrawal.

Returning families whose information has not changed and used the TMS plan last year do not need to complete a new TMS form. If you would like to stay in the TMS plan and are currently enrolled, check the TMS Rollover box on the registration form.

If you did not use TMS last year but would like to for the 2013-14 school year, please contact the Business Office (330.752.2505 or [email protected]) to get a TMS form. Registration packages for new families include a TMS form.

REGISTRATION FEE & TMS FEE

Registration fee must accompany your paperwork: - $300 per Returning Student ($450 after February 28, 2013) - $350 per New Accepted Student to the Elms

Do not include registration fees at this time for new siblings who have not received an acceptance letter for school year 2013-2014.

Families enrolling for PreK during the school year and summer PreK only pay one registration fee per year.

Note: Current students must be registered for school in order to register for classes in February.

REGISTRATION FEE (NON-REFUNDABLE)

You will be billed by TMS if you are enrolling in the TMS Tuition Payment Planfor 2013-2014.

Note: The $100 Elms Gala Family Obligation can be included in your TMS Plan. Please see page 5 for more information on the Elms Gala Family Obligation.

PLEASE NOTE:The non-refundable registration fee for returning students is $300 and is not included in the tuition listed above.

The non-refundable registration fee for new students is $350 and is not included in the tuition listed above.

The tax-deductible Elms Gala fee is $100 and is not included in the tuition listed above.

Senior graduation fee is included in tuition.

In addition to the tuition listed above, all returning student registrations received after the February 28, 2013 deadline will be charged a $150 late fee.

The PreK summer program must be paid in full prior to June 15, 2013 or one half by June 15 and the second half by July 13, 2013.*Please note: Students classrooms/teachers may be different for the PreK summer program.

SIBLING DISCOUNT (if requested by parent on Registration Form)The youngest student pays full tuition; the second student receives a $500 tuition discount; each additional student receives a $1,000 tuition discount.

Procedure for Elms Gala Fee (formerly known as February Fantasy).Each family is obligated to either pay $100, donate an auction item to the Elms Gala worth at least $100, or donate significant volunteer hours (to be determined by the School). In June 2013, the $100 obligation will be added to your tuition account. The $100 will be taken off of your account when your donated item is received or, if volunteering, within two weeks of the event. Otherwise, paying the $100 fulfills your family obligation. You do not need to notify us at this time which option you choose. You will not be penalized if you choose to volunteer or donate an item (for example, the School will not lock the family out of NetClassroom in January because your item has not been received.) However, if the School does not receive a donated item for Gala or have record of significant volunteer hours for Gala by the end of April 2013, you will be expected to pay the $100 obligation.

Medina/Brecksville/Broadview Hts/Strongsville/Northfield/Parma Transportation fee and billing to be determined by the companies in the spring.

Afterschool supervision is available for PreK, Elementary, Middle School, and High School students. Cost is $2.50 per half hour and billed on a monthly basis.

High School: Parents are billed each sports season. An Athletic Fee of $100 per fall and spring sport and $75 per winter sport is required of each athlete to defray uniform, equipment, transportation and other costs.

CYO: CYO fees vary by sport as listed on CYO registration documents.

ELMS GALA FAMILY OBLIGATION

There will be a $30 fee charged for every check made payable to Our Lady of the Elms School that is returned unpaid.

TUITION PAYMENT OPTIONSPlease indicate your choice of payment option on the Registration Form.

PAYMENT OPTION 1 - Payment in FullPAYMENT IN FULL of Tuition, Elms Gala Family Obligation (unless you chose other options) and if applicable, Chartered Bus/Van fee are due by August 1, 2013. Invoices will be mailed out in July 2013. Remit check payable to Our Lady of the Elms School or credit card information for the total amount due on or before Au-gust 1, 2013. Credit cards are accepted (Visa, MasterCard, Discover and American Express). A 3% handling charge will be added. Chartered Bus/Van fee/billing to be determined by the companies in the spring.

PAYMENT OPTION 2 - TMS Monthly PaymentsEnroll in the TMS Monthly Tuition Payment Plan by March 1, 2013. The TMS plan enables you to pay the year’s tuition, Elms Gala Family Obligation and, where applicable, Chartered Bus fee in:

- 12 monthly payments beginning June 2013 and ending May 2014 via TMS, the School’s agency for tuition collection. or - 10 monthly payments beginning August 2013 and ending May 2014.

Indicate your choice of plan on the Registration Form. This plan is not a loan and there are no interest charges. There is a per family administrative fee of $45 charged by TMS for automatic deduction from your bank account or monthly statements.

Registration fee is due at the time of enrollment. Tuition will be automatically rolled into your TMS plan. Registration fee is non-refundable.

Note: Returning families who used the TMS plan last year do not need to complete a new TMS form. If you would like to stay in the TMS plan and are currently en-rolled, check the TMS Rollover box on the registration form. If you were not on the TMS plan before and would like to be, please contact the Business Office to request a TMS form.

PAYMENT OPTION 3 - Two paymentsTwo payment plan allows payment of Tuition, Elms Gala Family Obligation (unless you chose other options) and if applicable, Chartered Bus/Van fee to be paid in two payments due by August 1, 2013 and January 2, 2014. Invoices will be mailed 4-6 weeks before payment is due.

Remit check payable to Our Lady of the Elms School or credit card information for the: - First half due on or before August 1, 2013 - Second half due on or before January 2, 2014

Credit cards are accepted (Visa, MasterCard, Discover and American Express). A 3% handling charge will be added. Chartered Bus/Van fee/billing to be determined by the companies in the spring.

TMS OPTION CHANGESIf you chose a different option from last year, please call the Business office at 330.752.2505 and request a TMS enrollment form.

FINANCIAL AID

Need-based financial aid is available for families with students in grades K-12. Financial aid forms (PSAS forms)are available online at www.TheElms.org. Forms are submitted to Private School Aid Service (PSAS). You must submit your 2012 W-2 and your most recent tax return. PSAS will objec-tively review all application forms and determine eligibility.

Tuition payments must be current in order for students to take semester and final examinations, to access NetClassroom and to register for the next school year. Transcripts and quarterly grade cards will be withheld for unpaid debts of tuition and fees (including Afterschool Program, non-returned library materials, and any athletic obligations).

Families of students registering at Our Lady of the Elms School after April 1, 2013 who wish to pay for tuition via the TMS Tuition Payment Plan, will not be able to take advantage of all 12 months of the payment plan. April 1, 2013 is the date by which we are required to submit enrollment materials to TMS for the 12-month plan. Parents of students registering after April 1, 2013, should contact the Business Office at 330.752.2505 or [email protected] to discuss how many months remain on the payment plan as of the date of their registration, how that will affect their tuition pay-ment, and how to expedite paperwork to take advantage of the time remain-ing on the plan.

After September 15 of each year, the responsible party for tuition of any full-time student at Our Lady of the Elms shall be responsible for the full year’s tuition. Should a student have to be withdrawn for any reason or is asked to leave, the entire tuition bill must be paid.

An appeal may be filed with the Finance Committee within 30 days of with-drawal. We have staffed our school based on your intent to attend all year and are still liable to pay our teachers for the full year.
