The following itets, at a minimum, shall be included with the petition for annexation:
!Z1. AnownershipplatmapfromUtahCountyshowingallpropertyownerswithinonthousand (1000) feet of the proposed annexation. If the petition is accepted for further review,
the applicant shall provide pre-addressed, stamped envelopes with the names of all property
owners within on thousand (1000) feet of the proposed annexation.
V 2. Signahires from the owners of real property located in the area proposed for annexation
that covers a majority of the private land and is equal in value to at least one third 1/3 of the
value of all private property, as shown by the latest tax assessment rolls of Utah County, in the
proposed annexation.
v' 3. An accurate map prepared by a licensed surveyor of the area proposed for annexation
suitable for recordation in the office of the County Recorder. Please identify on the plat each
parcel included in the annexation and on each parceI label, the owner's name, tax identification
number, acreage, and the proposed zoning. Two 36" x 24" and one 11" x 17" copy shall be
submitted to the Town, Mylar to be submitted prior to the final public hearing at City Council.
Y 4. The designation of up to five individuals, signatory to the petition, who shall serve as
sponsors of the annexation, together with the mailing addresses of each. Also, the designation of
one of the sponsors who shall act as "Contact Sponsor".
V 5. An agreement to observe and obey all applicable laws, ordinances, and resolutions
recognized by Elk Ridge City.
X6. The applicant shall indicate the proposed zoning designation of the area to be annexed.
V' 7. On the date of filing the petition with Elk Ridge City, the petition sponsors shall deliver
or mail a copy of the petition to the Utah County Recorder's Office and will produce evidence of
the mailing of delivery, whichever applies,

Ordinances in effect at this time. Should any of the information or representations submitted in
