Please Note: 1. The payment would be issued in the name of the Applicant as mentioned in company’s system.

2. If there is any discrepancy in the name provided (apart from name change) of the applicant, company will not enter such ECS form for online payment facility.

3. A blank cancelled cheque, with your name printed on cheque OR first page of pass book of the bank account mentioned above is mandatory to attach with this form.

4. If the name is not printed on the cheque leaf or it does not match with the name registered with the company, it is mandatory to get the bank details verified

by your banker.

5. If this form is received before the 20th of the month, the payment will be processed as per the information above for the same month. However, if this form

is received after the 20th of the month, it will be made effective from the following month onwards.

Online: If you are a Forever Business Owner who has access to you can update your bank

details online. ( Visit http://bitly.com/ECSOnline for online instructions)Step 1: Login with your registered email address and password at www.foreverliving.com, change location to Asia > India

Step 2: Go to “Store Profile” under the “My Account” menu.

Step 3: Go to “Bonus Paid Options” and select “Direct Deposit”. Once selected, enter the details in the field and “press the

save button”. Your details will be updated in our system.

Step 1: Fill up an ECS form available at any of our offices and attach a xerox copy of your cheque (blank / cancelled) with this

ECS form.

Step 2: Send / submit your ECS form at any of our offices.
