What is MTM?A group of pharmacists that work

together to help people get the most

from their medications.

• We talk to you about yourmedications and illnesses.

• We talk to your doctors to make surethe medications you take are correctand safe to take together.

• We keep track of what medicationsyou take, and give you an up-to-datelist at each visit.

• We help you talk to yourinsurance and get their medications.

You will work primarily with one

pharmacist who will get to know you

and the medications you are taking.

You can make an appointment with

your MTM pharmacist when you visit

your other doctors at UIC.

What will MTMdo for me?Your pharmacist will help you with your

medication and your health.

• My pharmacist will explain mymedications and illnesses. She will alsotalk about how my medicine shouldhelp me and explain possible sideeffects in a way that I can understand.

• My pharmacist will include my opinionin making decisions about my healthcare and the medicines I’m taking. Shewill talk to me before making changesto my prescriptions or treatment plan.

• My pharmacist will talk to all of mydoctors to make sure my medicines aresafe to take together.

• My pharmacist will respect me,listen to me, and keep my informationconfidential.

What do I need todo to be in theprogram?

• I will get all my prescriptions filled atUIC. If a prescription is needed whenthe pharmacy in not open I can goto another pharmacy to get themedication.

• I will ask my doctor for prescriptionsand bring them to my pharmacist.

• I will tell my pharmacist about allmedications that I am taking, includingvitamins, antacids, Tylenol, or herbals.I will also tell my pharmacist about anynew medications.

• I will contact the clinic if I need tochange my appointment. I understandthat if I do not come in on time I mayneed to wait to be seen.

• I will treat the MTM and pharmacystaff with respect and courtesy.
